门诊退药125例情况分析

目的分析门诊退药原因，并提出相应对策减少退药现象的发生，提高合理用药水平。方法对201l_02～2013—08期间收集的125例门诊药房退药情况进行回顾性总结分析，按退药原因、品种、科室及患者年龄分布分析退药现象的产生及特点。结果125例门诊药房退药处方中，患者申请退药理由主要为患者自身原因、药物不良反应和医生处方错误．三者共占比87．20％；涉及药物品种及其比例前4位分别有跌打类中成药（18．18％）、本院骨科制剂中成药（12．40％）、抗炎镇痛西药（11．16％）、抗感染类西药（8．68％）；产生退药科室集中在骨科中心、大内科、妇科与耳鼻喉科等门诊科室，共占比81．30％：退药患者年龄段集中在20。50a，占比71．20％。结论医院、医生、药师和患者多方共同努力．避免退药情况的发生。

Analysis of 125 Cases of Drug Repercussion in Out-Patient Pharmacy

Objective To analysis outpatient drug repercussion, and put forward the corresponding countermeasures to reduce the occurrence of the phenomenon of drug withdrawal, improve the level of rational use of drugs. Methods A total of 125 drug repercussion cases of out-patient pharmacy from February 2011 to August 2013 were collected. 125 cases of outpatient drug repercussion cases were retrospectively analyzed, according to the causes of drug returning, varieties, departments and the age distribution of patients of drug withdrawal phenomenon and characteristics. Results Of the 125 cases of drug withdrawal in outpatient pharmacy, the main reasons of drug withdrawal were as following： the patient＇s own reason, adverse reactions and drug prescribing errors, those 3 reasons accounted for more than 87.20% of all withdrawal. The first 4 involving drugs were traumatic medicines （ 18.18%）, medicine （12.40%） , anti-inflammatory withdrawal was concentrated in the medicine, department of gynecology, and analgesic medicine out-patient departments, as and department of ENT, those the hospital department of orthopedics preparation （ 11. 16%）, anti infective drugs （8.68%）. The department of orthopedics, department of internal accounted for more than 81.30%. Age of the patients with drug withdrawal was concentrated in 20-50 years old, accounted for more than 71.20%. Conclusion With joint efforts of hospital, doctors, pharmacists and patients, the occurrence of drug withdrawal cloud be reduced.
