吡格列酮对糖尿病大鼠肾脏的保护作用

摘    要:目的 :探讨胰岛素增敏剂吡格列酮对糖尿病大鼠肾脏的保护作用及其机制。方法 :将雌性SD大鼠 30只分为 3组 :正常对照组、糖尿病组和吡格列酮组。检测各组wk 4 ,8时血糖、糖化血红蛋白、肾功能、血脂 ,血转化生长因子 β1(TGF β1)水平等指标的变化 ,免疫组织化学检测纤维连接蛋白 (FN )和层粘连蛋白 (LN)表达水平。结果 :wk 8时吡格列酮组较糖尿病组尿蛋白排泄量明显减少 ,(2 0 .8±s 0 .7) μg·2 4h- 1vs (12 .6± 1.4 ) μg·2 4h- 1,P <0 .0 1;血TGF β1水平下降 (P <0 .0 1) ;wk 8时吡格列酮组肾小管FN和肾小球LN表达低于糖尿病组 ,P <0 .0 1和P <0 .0 5。结论 :吡格列酮对于糖尿病肾脏病变具有部分保护作用 ,可抑制血中TGF β1水平 ,减少细胞外基质的沉积。

Abstract:AIM: To investigate the protective effects of pioglitazone on renal and its mechanism in diabetic rats. METHODS: Thirty female SD rats were divided into the following three groups: normal control group,diabetic group (streptozotocin 70 mg·kg -1 ),pioglitazone group (pioglitazone 5 mg·kg -1 ·d -1,ig). Blood glucose,glycosylated hemoglobin,urea nitrogen,creatinine,urinary albumin,cholesterol, triglyceride,high density lipoprotein cholesterol,transforming growth factor beta (TGF-β_1) were measured after 4,8 wk treatment. Fibronectin(FN) and laminine (LN)expression were also measured by immuneohistochemistry. RESULTS: At 8 wk after treatment,urinary albumin ((20.8± s 0.7) μg· 24 h -1 vs (12.6±1.4) μg·24 h -1 , P < 0.01) and serum TGF-β_1 level of pioglitazone group were significantly lower than those of diabetic group ( P <0.01). The expressions of FN in the renal tube and LN in the glomerulus in the pioglitazone group were much lower than those in the diabetic group at wk 8( P < 0.01,P < 0.05 ). CONCLUSION: The results suggested that pioglitazone has some renal protective effects in diabetic rats,through decreasing serum TGF-β_1 level and reducing deposition of glomerular extracellular matrix.
