子宫颈鳞癌组织中MMP-7和CK20的表达及临床意义

目的 探讨基质金属蛋白酶-7(MMP-7)和细胞角蛋白20(CK20)在子官颈鳞癌中的表达特点及其相互关系,以及它们在子官颈鳞癌侵袭转移中的作用.方法 采用RT-PCR法检测33例子宫颈鳞癌和12例正常宫颈组织中MMP-7 mRNA和CK20 mRNA的表达水平,分析其表达水平与临床病理特征的关系.结果 MMP-7 mRNA在子宫颈鳞癌组织中的表达水平显著高于正常宫颈组织(P＝0.018),与子宫颈鳞癌分化程度、淋巴结转移显著相关(P<0.05),而与临床分期、年龄等因素无关(P>0.05);CK20 mRNA在子宫颈鳞癌组织中的表达水平高于正常宫颈组织(P=0.002),与子宫颈鳞癌分期、分化程度、淋巴结转移显著相关(P<0.05),而与年龄等因素无明显相关(P>0.05).在子宫颈鳞癌中MMP-7mRNA与CK20 mRNA的表达之间存在正相关关系(r=0.42735,P=0.0262).PCR法检测MMP7和CK20mRNA阳性率显著高于免疫组化法(P<0.05).结论 MMP-7 mRNA及CK20 mRNA的高表达与子宫颈鳞癌的侵袭转移有关,可作为检测早期官颈鳞癌淋巴转移的生物学指标.

Expression and clinical significance of MMP-7 and CK20 in cervical squamous cell carcinoma

Objective To explore the relationship between the expressions of MMP-7 and CK20 and cer vical squamous cell carcinoma,as well as the role of invasion and metastasis.Methods The MMP-7 mRNA and CK20 mRNA were detected by the RT-PCR method for 33 cases of squamous cell carcinoma of the cervix tissues and 12 cases normal cervix tissues.Results The expression of MMP-7 mRNA was significantly higher in cervical squamous cell carcinoma than that in normal cervical tissues(P=0.018).The obvious correlations were obtained between the expression of MMP-7 mRNA and the differentiation,as well as lymph node metastasis(P<0.05).But the significant relations were not found between the expression of MMP-7 mRNA and the clinical stages,age and other factors(P>0.05).The expression of CK20 mRNA was higher in cervieal squamous cell carcinoma than in the normal eervix tissues(P=0.002).The expression of CK20 was evidently correlated with the staging,of cervical squamous cell carcinoma,degree of differentiation and lymph node metastasis correlated(P<0.05).The significantly correlations were not found in other factors such as age(P>0.05).The significant positive correlation was found between the expressions of MMP-7 mRNA and CK20 mRNA in cervical squamous cell carcinoma tissues(r=0.42735,P=0.0262).The positive rate of MMP7 and CK20 mRNA detected by PCR were significantly higher than that by immunohistochemistry.Conclusion The expression of MMP-7 mRNA and CK20 mRNA are correlated with invasive and metastasis of cervical carcinoma.It maybe used as a biological indicator of metastasis for cervical squamous cell carcinoma in early.
