经尿道选择性绿激光治疗高龄高危良性前列腺增生症临床分析

目的：探讨经尿道选择性绿激光治疗高龄高危良性前列腺增生症的安全性、有效性。方法：回顾性分析采用经尿道选择性绿激光术治疗高龄高危良性前列腺增生症患者30例,观察平均手术时间、术后拔管时间、出血量、国际前列腺症状评分（IPSS）、剩余尿量、尿流率变化等指标。结果：30例患者均手术成功,手术时间平均46.3 min,术中出血量平均85.2 mL,术后留置导尿管时间平均24 h,IPSS评分由术前平均21.5分降至术后平均8.3分,最大尿流率由术前平均6.2 mL/s增加到术后平均18.5 mL/s,无1例出现手术相关并发症。结论：经尿道选择性绿激光治疗高龄高危良性前列腺增生症具有手术时间短、出血少、创伤小、恢复快、疗效确切等优点,扩大了高危患者手术的适应证。

A Clinical Analysis of Treatment of Benign Prostatic Hyperplasi Disorder in Senile and High -risk Patients with Transurethral Potassiumtitanylphosphate Laser

Objective： To explore and assess the safety and efficacy of transurethra potassiumtitanylphosphate laser therapy in senile and high-risk patients with benign prostatic hyperplasia disorder.Methods： 30 cases of benign prostate hyperplasia disorder treated with transurethra potassiumtitanylphosphate laser were retrospectively analyzed,examining such indicators as the average operation time,postoperative decannulation time,blood loss,international prostate symptoms score（IPSS）,residual urine output and changes in urine flow rate.Results： The operation was a success in all 30 patients,with an average operation time of 46.3 min,an average peri-operative bleeding volume of 85.2 mL,an average postoperative lien time of 1 day,a fall in IPSS From 21.5 points on the average preoperatively to 8.3 points on the average postoperatively and an increase in the urine flow maximum from 6.2 mL/s on the average preoperatively to 18.5 mL/s on the average postoperatively.No surgery-associated complication occurred.Conclusion： Transurethra potassiumtitanylphosphate laser therapy of benign prostatic hyperplasia in senile and high-risk patients has such advantages as short operation time,less bleeding,small trauma,quick recovery,definite effects,etc,and thus can expand surgical indication in high-risk patients.
