复发与未复发女性尖锐湿疣患者Th_1/Th_2细胞因子检测

摘    要:
目的观察复发与未复发女性生殖器尖锐湿疣(CA)患者外周血Th1/Th2细胞因子水平,探讨CA复发与Th1/Th2细胞因子的相关性。方法选取女性生殖器CA初发患者37例,经电灼治疗后随访3个月(期间不使用任何免疫制剂),再次复发者计入A组(15例),未复发者计入B组(22例)。用ELISA法测定电灼治疗前各组血清标本的细胞因子IL-2、IL-12、IL-4和IL-10的浓度,比较A,B两组血清细胞因子及Th1/Th2比值的差异,分析CA复发与Th1/Th2细胞因子水平的相关性。结果A组治疗前的血清IL-2,IL-12,IL-4,IL-10分别为65.86±20.31,21.09±5.84,98.31±10.95,25.87±9.24,B组分别为82.81±27.24,25.74±10.63,89.02±13.80,25.62±9.84;用Th1/Th2比值来预测CA不复发的敏感性为86.4%,特异性为80.0%,ROC曲线下面积为0.812。结论CA复发与否与患者血清Th1/Th2细胞因子水平呈负相关,以Th1/Th2比值预测CA复发与否有较高的敏感性和特异性,Th/Th比值可作为评价CA复发与否的一个参考指标。

Abstract:
Objective To observe the level of Th1/Th2 cytokines in relapse of CA and investigate the role of Th1/Th2 cytokines in relapse of female genital condyloma acuminate. Methods 37 primary infection of female genital CA were enrolled within 3 months following up after electric cautery and no immunity treatment. They were divided into two groups according relapse or not, including 15 cases of group A (which are relapse) and 22 casea of group B (which are not relapse). level of interleukin (IL)-2, IL-12, IL-4 and IL-10 were assayed by ELISA and the relationship between serum levels of Th1/Th2 cytokines and relapse of CA was analyzed. Results Before treatment, level of interleukin (IL)-2, IL-12, IL-4 and IL-10 in sera were 65.86±20.31,21.09±5.84,98.31±10.95,25.87±9.24 respectively in group A and were 82.81±27.24,25.74±10.63,89.02±13.80,25.62±9.84 respectively in group B. The sensitivity was 86.4% and the specificity was 80.0%.The area under the ROC curve was 0.812 with Th1/Th2 ratio predicting CA no relapse. Conclusion Relapse of CA was significantly correlated with the levels of Th1/Th2 cytokines. Ratio of Th1/Th2 had higher sensitivity and specificity to predict CA with no relapse.
