皮瓣牵拉、固定预防乳腺癌手术后皮下积液和皮缘坏死

目的:评价皮瓣牵拉、固定方法在预防乳腺癌改良根治术后皮下积液与皮缘坏死的作用。方法:乳腺癌改良根治术后应用皮瓣牵拉、固定方法缝合245例,观察皮下积液与皮缘坏死的发生率及其严重程度,并和前期272例乳腺癌改良根治术后常规缝合的皮下积液与皮缘坏死的发生率相比较。结果:应用皮瓣牵拉、固定方法的245例患者皮下积液发生率3.27%,皮缘缺血、坏死发生率7.35%。常规缝合的272例患者皮下积液发生率12.13%;皮缘缺血、坏死发生率18.75%。经统计学检验表明:两组的皮下积液发生率和皮缘缺血、坏死发生率具有显著性差异(P<0.001,P<0.001)。结论:皮瓣牵拉、固定方法可有效预防乳腺癌改良根治术后皮下积液与皮缘坏死。

The clinical effect of flap suture and fixation on the prevention of seroma and flap margin necrosis after modified radical mastectomy of breast cancer

Objective: To evaluate the effect of flap suture and fixation on the prevention of seroma and necrosis of flap margin after modified radical mastectomy of breast cancer. Method: The flap of 245 cases of breast cancer after modified radical mastectomy were sutured and fixed, and the rate of seroma and margin necrosis of flap were observed and compared with 272 cases underwent modified radical mastectomy without flap suture and fixation. Result: The rate of seroma and flap margin necrosis of flap suture and fixation group was 3.27% and 7.35% respectively, and that of control group was 12.13% and 18.75% respectively, and difference between them showed statistical significance (P<0.001;P<0.001 respectively). Conclusion: The method of flap suture and fixation showed good effect in preventing the occurrence of seroma and flap margin necrosis.
