三种光敏剂介导的光动力疗法治疗上消化道肿瘤的疗效比较研究

目的分析比较三种光敏剂介导的光动力疗法治疗上消化道癌的近期疗效和不良反应。方法上消化道癌患者181例，分别应用血卟啉衍生物（67例）、血卟啉单甲醚（58例）及卟吩姆钠（56例）作为光敏剂，给药剂量分别为5、6和2mg／kg，于照光前6—24h静脉滴注。分别应用金蒸气激光和半导体激光作为治疗光源，功率密度200～400mW／cm^2，能量密度100～300J／cm^2。治疗1-5次后评价疗效：完全疗效（cR）、明显疗效（PR）、稍有疗效（MR）、无效（NR）。结果血卟啉衍生物组67例患者，cR5例（7．5％），PR40例（59．7％），MR16例（23．9％），NR6例（9．0％）。血卟啉单甲醚组58例患者，CR5例（8．6％），PR33例（56．9％），MR14例（24．1％），NR6例（10．3％）。卟吩姆钠组56例患者，CR6例（10．7％），PR33例（58．9％），MR12例（21．4％），NR5例（8．9％）。三组病例均未出现严重不良反应。结论血卟啉衍生物、血卟啉单甲醚和卟吩姆钠三种光敏光动力疗效治疗临床各期上消化道癌疗效相近，不良反应轻，耐受性好，是较好的姑息治疗手段。

Three Different Photosensitizers for Upper Gastrointestinal Tract Cancers: a Comparative Study
