摘 要: | 目的探讨多普勒超声测量正常儿童肾动脉血流参数的临床价值,为临床儿童肾脏疾病诊治提供参考。方法选取300名体检正常的健康儿童,采用彩色多普勒超声测量主肾动脉(MRA)、段动脉(SA)和叶间动脉(ILA)的频谱,计算舒张末期峰值(Vd)、收缩期血流峰值流速(Vs)、平均血流速度(Vm)、收缩期血流峰值流速与舒张末期血流峰值流速比值(S/D)、阻力指数(RI)、搏动指数(PI)、收缩期血流加速时间(AT)等。结果正常儿童肾动脉血流参数与体表面积(BSA)之间具有良好相关性,Vd、Vs、Vm与BSA间呈正相关(均P〈0.05),S/D、RI、PI与BSA间呈负相关(均P〈0.05);不同肾动脉分级血流参数中Vd、Vs、Vm、S/D、RI、PI比较差异具有统计学意义(P〈0.05),MRA、SA、ILA呈现依次递减趋势,AT比较差异无统计学意义(P〉0.05)。结论多普勒超声测量正常儿童肾动脉血流参数结果与儿童BSA之间具有良好相关性,可为临床儿童肾脏疾病诊治提供参考。

Abstract: | Objective To explore the Doppler ultrasound measurement of the clinical value of renal artery blood flow parameters of normal children in order to provide reference data for clinical diagnosis and treatment of kidney disease in children. Methods Three hundred healthy children undergoing physical examinations were selected and the main renal artery( MRA),segmental artery( SA),and leaf between artery( ILA) spectrum were measured with color Doppler ultrasound. The end-diastolic peak( Vd),systolic peak velocity( Vs),mean blood flow velocity( Vm),systolic peak velocity and peak end-diastolic velocity ratio( S /D),resistance index( RI),pulse index( PI),systolic acceleration time( AT) blood flow were calculated. Results Renal artery blood flow parameters of normal children had a good correlation with the surface area( BSA). Vd,Vs,Vm had a positive correlation with BSA( allP 0.05),and S/D,RI,PI had a inverse correlation with BSA( allP 0.05). Comparison of different classified renal artery blood flow parameters of Vd,Vs,Vm,S /D,RI,PI showed statistically significant differences( P〈0.05),magnetic resonance imaging( MRI),SA,ILA presented a decreasing trend and there was no statistical difference in AT comparison( P〉0.05). Conclusion The result of Doppler ultrasound during normal pediatric renal artery blood flow parameter measurement has a good correlation with BSA of children and can provide reference for clinical diagnosis and treatment of kidney disease in children. |
