双胎妊娠生长发育不一致的预后分析

目的研究并分析双胎妊娠生长发育不一致及孕产妇的预后情况。方法收集2005年9月～2010年9月住院分娩的90例双胎妊娠生长不一致孕产妇（研究组）的临床资料,并与90例双胎妊娠生长发育一致孕产妇（对照组）的新生儿结局以及孕产妇的围生期情况进行比较。结果研究组新生儿体重明显低于对照组（P〈0.05）。研究组围生儿死亡率为21.1%（19/90）,明显高于对照组的3.3%（3/90）,差异有统计学意义（P〈0.05）。研究组孕产妇分娩孕周明显低于对照组（P〈0.05）。研究组围生期双胎输血综合征、晚期流产、羊水过多、胎盘早剥、胎儿畸形、早产的发生率明显高于对照组（P〈0.05）。结论双胎妊娠生长不一致性的并发症发生率、新生儿患病率和死亡率均明显高于妊娠生长发育一致性双胎。早发现、早诊断、及时干预是降低新生儿不良结局的重要手段。

Prognosis analysis of twin pregnancy with inconsistent development

Objective To research and analyze prognosis of neonates and pregnant and lying-in women with twin pregnancy of inconsistent development. Methods Ninety pregnant and lying-in women with twin pregnancy of inconsistent development hospitalized in our hospital from September 2005 to September 2010 were chosen as research group while 90 pregnant and lying-in women with twin pregnancy of consistent development in our hospital from the same period of time were chosen as control group. Neonatal outcomes and conditions of maternal perinatal period were compared between these two groups. Results The neonatal weight of research group was significant lower than control group（P 0.05）. Neonatal mortality rate of research group was 21.1% （19/90）, it was higher than control group3.3% （3/90）], the difference was statistically significant （P 0.05）. Delivery gestational weeks of research group were less than control group（P 0.05）. The incidence of twin transfusion syndrome, late abortion, hydramnios, placental abruption, fetal abnormalities, preterm birth of research group were significantly higher than control group （P 0.05）. Conclusion The incidence of complications, neonatal morbidity and mortality of twin pregnancy with inconsistent development are significantly higher than twin pregnancy with consistent development. Early detection, early diagnosis and timely intervention are vital means to reduce the rate of adverse neonatal outcomes.
