ICU双套结在经口气管插管中固定法的临床研究

目的探讨经口气管插管的优化固定方法。方法使用纱布绷带打成双套结，固定气管插管。分为两个时间段来研究：第1阶段为2012年1～3月，28例气管插管患者采用传统的胶布固定方法，为对照组；第2阶段为2012年4～6月，28例使用双套结固定气管插管，为实验组。对两组气管插管后意外拔管率．、患者舒适度、皮肤受损情况等方面进行比较分析。结果通过使用双套结固定气管插管的优化固定方法，与对照组相比，实验组意外拔管率下降，固定效果好，患者舒适满意度高，固定方法操作简便，口腔溃疡和压疮发生率降低，减少了护理工作量和节约了医疗费用。与对照组比较差异有统计学意义。结论双套结固定经口气管插管的方法，减少了非计划性拔管，提高了患者舒适度，是经口气管插管患者安全的一个重要保证。

Clinical research of two bowline fixation method of intubation in ICU

Objective To discuss about the optimized lixatlon method ot mtubatlon, lvletnofls lntuDauon tune was fixed by two bowline gauze. Research was mainly done in two period of time： 28 eases of intubation were fixed by tra- ditional tape fixation as control group from January to March 2012, and 28 eases of intubation were fixed by two bow- line gauze fixation as experiment group from April to June 2012. Items like accidental extubation rate, patient comfort degree, skin damage degree were compared between ihe two groups. Results The statistically significant differences existed in accidental extubation rate, patient comfort degree and other fixation effects which experiment group showed better results. Two bowline fixation method lowered mouth ulcers, pressure sores, attendance and medical cost with simple opration. Conclusion Two bowline fixation method of intubation is an important secure to patients＇ safety which lowered the rate of accidental extubation, enhanced patients＇ comfort degree.
