阴道镜对诊断宫颈病变的临床分析

目的:探讨阴道镜检查对宫颈病变的诊断的临床价值。方法:研究对象为2003年1月1日至2004年12月31日陕西省妇幼保健院妇产科诊治的3 541例患者,对其宫颈阴道镜检查结果及镜下定位活组织病理检查结果进行分析。结果:经病理确诊为宫颈上皮内瘤样病变(CIN)的患者287例,其中CINⅠ228例,CINⅡ29例,CINⅢ(包括宫颈原位癌CIS)30例;宫颈浸润癌87例,慢性宫颈炎1 899例,亚临床人乳头状病毒感染(SPI)147例,宫颈息肉461例,子宫内膜异位症12例,平滑肌瘤14例。结论:阴道镜检查对早期发现宫颈癌,宫颈癌前病变、亚临床人乳头状病毒感染(SPI)有重要价值。对CIN诊断的敏感性和阴性预测值高。统一阴道镜评分标准的意义重大。

Clinical Analysis of Colposcopy in the Diagnosis of Cervical Disease

Objective To evaluate the value of colpscopy on the clinical diagnosis of cervical disease.Methods Colposcopy was used to analyse the summarize 3541 cervical cases in the Mother and Child's Hospital of Shannxi Province from Jan.2003 to Dec.2004.Results Of all 3541 cases,women were hist opathologically confirmed suffering with CIN 287 cases,include CINⅠ228 cases,CINⅡ29 cases,CINⅢ(include CIS)30 cases; with chronic cervicitis 1899 cases;with subclinnical papillomavirus infection 147 cases;with polyps 461 cases and with others 26 cases;12 cases cervical endometriosis;14 cases leiomyoma.Conclusions Colposcopy have a very important value in finding early stage carcinoma of cervices,CIN and SPI.It is quite important to the unify the colposcopic grading index.
