摘    要:目的研究间歇性充气加压对骨折愈合的影响。方法26只新西兰大白兔于右侧胫骨中段造成3mm缺损,术后长腿管型石膏外固定,随机分成A、B2组。术后1周拆除管型石膏后予长腿石膏夹板外固定,对A组截骨部位进行间歇性充气加压,每次30min,每天2次,持续8周;B组带不予充气的气囊作为对照组。术后每2周X线摄片和骨密度(BMD)测试。第10周A、B2组各随机取3只兔子处死,行组织学检查骨痂面积、成骨细胞数。其余兔处死后取双侧胫骨作机械力学测试。结果A组的骨痂面积、成骨细胞含量同期均高于B组。治疗后2组间和组内比较差异均有显著性,2组BMD和BMD比率在术后均随时间延长而增加,A组8周时BMD含量已达健侧胫骨同一部位的水平,4、6、8、10周时A组BMD和BMD比率高于B组(P<0.05,P<0.01)。A组力学测试的4项参数平均化为对侧完整胫骨参数值的百分比后比B组高,差异有显著性(P<0.05,P<0.01)。结论间歇性充气加压可促进骨折的愈合,减轻软组织水肿,防止骨缺失,增加骨折愈合后的机械强度。

Abstract:Objective To study the effect of intermittent pneumatic compression on fracture healing.Methods A 3 mm gap was performed in right mid-tibials of 26 New Zealand white rabbits. The osteotomy site was stabilized with long leg plaster cast external fixation postoperative.The rabbits were divided randomly into two groups such as A and B group. On postoperative a week,the external fixation was removed.Then they were fixed with long leg plaster splint.The A group was treated with intermittent pneumatic compression at midpart of the calf. The treated was done for 30 min twice a day for 8 weeks. The B group was not treated as the control group.Fracture healing was assessed with X-ray and bone mineral density test per two postoperative weeks. At 10 th week of postoperation, the rabbits 3 of each group were killed of histologic analysis was tested with callus area and the quantity of osteoblast.Both tibias of the remine were transected for biomechanical testing.Results Callus area , the quantity of osteoblast in the A group were more than the B group.There are difference between two groups and interblock.Bone mineral density and bone mineral density ratio at the ostectomy of two groups increased along with times.At the 8th postoperative week in A group,the BMD of the ostectomy was as same as the nomal. BMD and BMD ratio at the ostectomy increased significantly,it was much more in the A group than that in the B group at 4,6,8,10 week(P<0.05,P<0.01).4 mechan-parameters of A group were also much more than the B group when they were normalized as a percentage of the value for the contralateral, intact tibial. The difference was significant(P<0.05,P<0.01).Conclusion IPC can promote the fracture healing, lighten soft tissue edema, prevent bone loss, and improve the biomechanical properties of a healing osteotomy site.
