前臂游离皮瓣在全鼻再造中的应用

目的总结前臂游离皮瓣应用于全鼻再造的经验。方法应用前臂游离皮瓣进行全鼻再造7例,桡动脉与面动脉吻合5例,与甲状腺上动脉吻合2例;头静脉与颈外静脉吻合6例,与面前静脉吻合1例。结果 7例全鼻再造手术均一次完成,除1例因皮瓣青紫行手术血肿探查并清除颈部血肿,其余无并发症发生,皮瓣一期成活,鼻外形满意。结论前臂皮瓣全鼻再造,具有血管恒定,管径较粗大,供皮瓣面积大,吻合易于成功,不造成面部或邻近供区的损伤,皮瓣质软,弹性好,且与面部颜色相差不十分明显等优点,具有较好的临床应用价值。

Experiences of nasal reconstruction with forearm free flap

Objective To explore the experience of nasal reconstruction with forearm free flap.Nasal re-construction with forearm free flap were applied in seven cases,including radial artery and facial artery anastomosis,cephalic vein and external jugular vein anastomosis.These forearm free flaps were alive in all patients,all incision healed in I stage,no post opera-tive complications were found.The shapes of nose were satisfactory.The method can be clinically applied for its simple procedure,reliable flap′s blood supply,high survival rate and satisfied result.
