踝关节色素沉着绒毛结节性滑膜炎早期诊断与治疗

[目的]探讨踝关节色素沉着绒毛结节性滑膜炎的早期诊断与手术疗效。[方法]回顾性分析5例踝关节色素沉着绒毛结节性滑膜炎患者资料，左侧2例，右侧3例；年龄18～50岁；病程2～12a。5例早期均被误诊。均行滑膜切除术后放射治疗。[结果]随访1～4a，平均2．5a，治愈2例，好转3例，未见复发。[结论]该病早期诊断困难，临床、放射学及病理检查相结合是确诊关键；滑膜切除术后结合放射治疗疗效确切。

Diagnosis and treatment of pigmented villonodular synovitis of the ankle joint

[ Objective ] To explore early diagnosis and operative treatment of pigmented villonodular synovitis of the ankle joint. [ Method ] Five patients with pigmented villonodular synovitis of the ankle joint were reviewed, of which 2 cases in left and 3 in right. The age was from 18 to 50 years. All patients had not been previously diagnosed. Five patients had surgical excision followed by radiation therapy with dosages ranging from 3200 -4000 cGy. [ Result] Five patients were followed up for 1 - 4 years. No recurrence was noted at a mean followup of 2. 5 years for primary lesions. The results were excellent in 2 cases, good in 3. [ Conclusion] It is difficult for diagnosis in early stage of pigmented villonodular synovitis of the ankle joint and combination of clinical observation, radiological examination and pathological examination is essential for confirmed diagnosis. Surgical excision of primary lesions with postoperative radiation are recommended.
