偏头痛患者的性格、情感与认知功能改变

目的：用国内通用的心理量表及事件相关电位P300检测偏头痛是否有性格特点、情感改变及其认知功能变化,观察三之间的相互关系。方法：32例无先兆偏头痛患于发作间歇期,进行艾森克个性问卷(EPQ)、A型行为量表、焦虑自评量表(SAS)、抑郁自评量表(SDS)和听觉P300检查,以32例健康人作对照。结果：偏头痛患EPQ的神经质分(N)、A型行为量表的CH分及SAS,SDS的得分均显高于对照组;P300中的N2波及P3波的波幅低于对照组,各波的潜伏期及其余各波波幅均正常。结论：偏头痛具有明显性格特征及情感障碍,且有轻度认知功能障碍。因前二之间关系密切,而认知障碍与性格和情感改变无平行关系,故考虑情感改变可能是偏头痛发作的因素之一。而认知障碍则可能为头痛的结果。研究表明应重视偏头痛防治中的心理因素。

Changes of personality, emotion and cognitive function in migraineurs

AIM: To investigate the personality, emotion and cognitive function of migraineurs using the psychological assessments and P300. METHODS: Thirty two migraineurs without aura and 32 age and sex matched normal controls were examined by the psychological assessments and P300, including Eysenck Personality Questionnaire (EPQ), Jenkins Activity Survey (JAS) and Self rating Anxiety (SAS) and Self rating Depression (SDS) Scales. RESULTS: The migraineurs had significantly higher scores on SAS, SDS, the neuroticism of EPQ and CH of JAS. Lower amplitudes of N2 and P3 in auditory P300 were also found. CONCLUSION: Specific personality and emotional changes does exist in migraine. Besides, mild cognitive changes are also present. Because of the fact that the personality and emotional changes are closely related with each other while the cognitive dysfunction is not parallel with them, the emotional changes are considered as a part of the causative factors for the migraine attacks while the cognitive dysfunction, most probably, is the result of headache. The study indicates that the psychological factor should be emphasized in the management of migraine
