四项血清肿瘤标志物联合检测和ROMA指数在卵巢癌中的应用价值

目的 探讨四项血清肿瘤标志物HE4、CEA、CA125、CA72-4的联合检测和ROMA指数在卵巢癌中的应用。方法 选择55例卵巢癌患者作为恶性组,61例卵巢良性肿瘤患者作为良性组,60例体检合格的健康人群作为对照组,检测三组的HE4、CEA、CA125和CA72-4水平,并利用HE4和CA125的值计算ROMA指数,比较三组上述参数的水平、阳性率、灵敏度及特异度。绘制ROC曲线以评价上述四项血清肿瘤标志物单项和联合检测及ROMA指数对卵巢癌的诊断价值。结果 恶性组中的HE4、CEA、CA125、CA72-4的单项及联合检测和ROMA指数水平及阳性率均高于良性组和对照组,差异有统计学意义(P＜0.05)。HE4、CEA、CA125和CA72-4四项肿瘤标志物联合检测后,灵敏度由单项CA125最高54.5%提高到78.2%,再联合ROMA指数,灵敏度进一步提高,为92.7%。对ROC曲线分析,HE4、CEA、CA125、CA72-4和四项联合检测及ROMA指数的曲线下面积分别为0.820、0.706、0.774、0.736、0.839、0.880,可知ROMA指数对卵巢癌诊断的准确性最好。结论 四项血清肿瘤标志物的联合检测以及ROMA指数对卵巢癌的诊断具有重要意义。

The application value of four serum tumor markers and theROMA index in ovarian cancer

Objective To explore the application of four serum tumor markers HE4,CEA,CA125,CA72-4,and ROMA index in ovarian cancer.Methods Choice of 55 patients with ovarian cancer as malignant group,61 cases of patients with ovarian benign tumor as benign group,60 cases of medical examination of healthy people as control group,three groups of HE4,CEA,CA125 and CA72-4 levels,and uses of HE4 and CA125 value to calculate an index,compare three groups the levels of these parameters,the positive rate,sensitivity and specific degrees.ROC curve was drawn to evaluate the value of single and combined detection of the above four serum tumor markers and ROMA index in the diagnosis of ovarian cancer.Results Malignant group,HE4,CEA,CA125,CA72-4 levels and the positive rate of individual and joint detection,the ROMA index level and positive rate,were higher than in benign the four tumor markers of HE4,CEA,CA125 and CA72-4,the sensitivity was increased to 78.2 percent from 54.5 percent of the single-item CA125,and the sensitivity was further improved to 92.7% by combined ROMA.According to the ROC curve analysis,HE4,CEA,CA125,CA72-4,4 joint detection and the ROMA index,the area under the curve were 0.820,0.706,0.774,0.736,0.839,0.880,shows the ROMA index is ovarian cancer diagnosis accuracy index is best.Conclusion The combined detection of four serum tumor markers and the ROMA index are important for the diagnosis of ovarian cancer.
