输卵管子宫内膜异位症21例临床分析

目的 :探讨输卵管子宫内膜异位症 (TEM)的临床特征及可能发病的有关因素。方法 :回顾分析 2 1例TEM患者的临床资料。结果 :输卵管浆膜肌层子宫内膜异位 6例 ,输卵管粘膜肌层子宫内膜异位 15例 ;TEM合并输卵管积水 8例。TEM无其他盆腔子宫内膜异位病灶 13例 (13/ 2 1) ,其中有输卵管结扎手术史者占 84 .6 2 % (11/ 13)。TEM结合其他盆腔子宫内膜异位病灶临床分期 ,Ⅰ期 10例 ,Ⅱ期 7例 ,Ⅲ期 1例 ,Ⅳ期 3例 ;而单纯为TEM临床分期 ,Ⅰ期 17例 ,Ⅱ期 4例。结论 :输卵管结扎术可能是诱发TEM的高危因素 ;子宫输卵管造影术有助于TEM的诊断 ,但确诊仍需要病理检查 ;治疗TEM可选择保守性手术治疗

Clinical analysis of 21 cases with tubal endometriosis

Objective:To investigate the clinical characteristics and etiological factors of tubal endometriosis(TEM).Methods:Retrospective analysis of 21 cases with TEM was carriedout.Results: Six cases with subserous salpinx endometriosis,15 cases with mucosal and muscular oviduct endometriosis and 8 TEM cases complicated withhydrosalpinx were found respectively.Thirteen out of 21 TEM cases were found without any other pelvicendometriosis and 84.62% of TEM cases had past tubal sterillization(11/13).Ten cases,7 cases,1 case and 3 cases were for stage Ⅰ,Ⅱ,Ⅲ and Ⅳ respectively when staging with other pelvic endometriosis.Seventeen cases and 4 cases were for stage Ⅰ and Ⅱ respectively when staging without any other pelvic endometriosis. Conclusions:Tubal sterillization may be a high risk factor thatmakes TEM attack.Hysterosalpingography may benefit to TEM diagnosis,but final diagnosis only depends on pathological examination.Conservative operation may be useful for TEM treatment.
