肝硬化患者血小板参数和凝血指标的观察

[目的]了解肝硬化患者血小板及其相关参数和凝血指标的变化,探讨其临床意义。[方法]检测62例肝硬化患者血小板参数(PLT、PCT、MPV、PDW)、凝血指标(PT、TT、APTT、FIB),并与50例健康对照组的血小板及其相关参数和凝血指标进行比较,同时肝功能分级后组间血小板及其相关参数也进行了比较。[结果](1)肝硬化组与正常对照组比较,PLT、PCT显著下降,MPV、PDW显著升高;肝功能B、C级与A级比较,C级PLT和PCT下降显著,MPV、PDW显著升高。(2)肝硬化患者PT、TT、APTT均较对照组明显延长,FIB明显降低。[结论]肝硬化患者存在血小板及凝血异常,血小板参数和凝血指标可以反映肝病的肝损程度和病情轻重,指导临床治疗及其预后都具有重要的意义。

SURVEY OF PLATELET PARAMETERS AND COAGULATION INDICES IN PATIENTS WITH HEPATIC CIRRHOSIS

[Objective] To analyze the change of platelet parameters and coagulation indices in patients with hepatic cir-rhosis. [Methods] The platelet parameters (PLT, PCT, MPV, PDW) and coagulation indices (PT, TT, APTT, FIB) of 62 cases of hepatic cirrhosis and control group were examined and analyzed by t test. [Results] (1) Compared with the con-trol group, PLT, PCT descended evidently and MPV, PDW increased significantly. As the pathogenetic condition was more se-rious, PLT, PCT were lower and MPV, PDW were higher on B and C stage patients with hepatic cirrhosis than A stage ones. (2) PT, TT, APTT in 62 cases of patients were longer than in the control group and FIB descended evidently, and there was obviously different between two groups. [Conclusion] There is abnormality of platelet parameters and disturbance of blood co-agulation in patients with hepatic cirrhosis, some of platelet parameters and coagulation indices may reflect the pathogenetic condition and damage degree of liver disease.
