摘    要:
目的探讨后循环缺血患者的脑血流动力学听觉诱发电位改变。方法对110例经临床确诊的后循环缺血(PCI)患者依据临床症状及CT/MRI检查结果分为梗死组(48例)和非梗死组(62例),均进行脑干听觉诱发电位(BAEP)检查,并与50例健康对照组进行对比。结果 110例PCI患者BAEP异常86例,异常率78.2%,主要异常表现为:Ⅰ、Ⅲ、Ⅴ波的潜伏期和Ⅲ~Ⅴ潜伏间期延长,表现以脑干型为主(33/110,30%),高于内耳型(24/110,21.9%)和混合型(29/110,26.4%)。梗死组Ⅲ波潜伏期(4.12±0.23)较非梗死组(3.98±0.21)显著延长(P<0.05),梗死组Ⅴ波潜伏期(6.04±0.25)较非梗死组(5.75±0.39)显著延长(P<0.01),梗死组Ⅲ~Ⅴ潜伏间期(2.08±0.23)较非梗死组(1.77±0.31)显著延长(P<0.01)。结论BAEP能灵敏地检测出后循环缺血患者的神经电生理异常,其异常程度可部分反应病情的严重程度,对后循环缺血的诊断和病情评估,提供重要的参考。

Abstract:
Objective To investigate the changes of Electroneurophysiology in patients with posterior circulation ischemia(PCI).Methods 110 patients with PCI which been divided in infarct group(48 cases) and non-infarct group(62 cases) were evaluated by brain auditory evoked potential(BAEP)compared with controls(50 cases).Results In all 110 PCI patients the abnormalities of BAEP were detected in 86cases(78.2%).The main abnormal findings of BAEP included prolonged peak latency(PL) of Ⅰ.Ⅲ.Ⅴ waves and prolonged interpeak latency(IPL) of Ⅲ~Ⅴ wave.The brain stem type(33/110,30%)abnormality was higher than the type of inner-ear type(24/110,21.9%)and the mixied type(29/110,26.4%).The PL of Ⅲ wave in infarction group(4.12±0.23) was significantly prolonged than non-infarction group(3.98±0.21)(P<0.05);The PL of Ⅴ wave in infarction group(6.04±0.25) was significantly prolonged than non-infarction group(5.75±0.39)(P<0.01);The IPL of Ⅲ~Ⅴ wave in infarction group(2.08±0.23) was significantly prolonged than non-infarction group(1.77±0.31)(P<0.01).Conclusion BAEP could detected the electro neurophysiologic abnormalities sensitively in patients with PCI,BAEP had played a very important role on the early diagnosis,severity assessment and prognosis indicate in PCI.
