前列腺切除术后并发症的防治

目的探讨耻骨上前列腺切除术后常见的出血、膀胱无抑制痉挛、尿失禁、膀胱颈狭窄、伤口感染5种并发症的原因及防治方法。方法采用深缝扎前列腺窝5、7点处血管及术后导尿管牵拉压迫;术中避免损伤膀胱外括约肌;缝合膀胱颈口时,可容纳导尿管及1指大小;术后由骶管内经微量泵持续注入吗啡、氟哌利多、布比卡因;术后切口有效引流。结果 121例患者采用上述措施治疗,取得较好的临床效果。结论采用上述措施,能有效防治上述5种并发症。

Management of Postoperative Complication after Transurethral Resection of the Prostate

Objective To investigate the causes and prevention of complications which contain bleeding,bladder inhibition spasms,urinary incontinence,bladder neck stenosis,wound infection after suprapubic prostatectomy Methods Circular deep suture of the prostatic vessels during operation and by Foley catheter traction or transurethral electric coagulation during operation;avoid injuring the external compression during operation;maintain the reconstructed internal urethral orifice big enough to contain about 1 finger;injecting pain medication persistently by micro pump;effective drainage.Results The methods were applied in 121 patients successfully.Conclusion The above methods are effective in management of postoperative complication.
