支气管哮喘患儿血清嗜酸性粒细胞阳离子蛋白和总免疫球蛋白E与肺功能关系的研究

摘    要:目的探讨不同时期、不同程度的支气管哮喘患儿血清嗜酸性粒细胞阳离子蛋白(ECP)和血清总免疫球蛋白E(TIgE)的变化,及其与肺功能各指标之间的关系。方法测定53例支气管哮喘患儿急性发作期和缓解期血清TIgE和ECP水平,同时作肺功能检测,并设20例健康正常儿作对照。结果支气管哮喘患儿急性发作期血清ECP水平明显高于缓解期和对照组(P<0.01),不同程度哮喘患儿在急性发作期血清ECP水平存在显著性差异,在缓解期则无显著性差异;TIgE水平在急性发作期和缓解期均高于对照组(P<0.01),但在急性发作期和缓解期及不同发作程度之间无显著性差异。血清ECP水平与肺功能指标FEV1的实测值及百分比(FEV1%pre)、1 s用力呼气容积与用力肺活量比值(FEV1/FVC)呈负相关;TIgE与肺功能无相关性。结论ECP水平增高与肺功能减退、呼吸道反应性增高和哮喘发作有直接关系,血清ECP水平能准确反映患儿嗜酸粒细胞活化状态和气道炎症变化,是诊断、病情判断及指导治疗的可靠指标。

Abstract:Objective It is to discuss the relationship between the changes of serum eosinophile cationic protein(ECP) and serum total immunoglobulin E(TIgE) and various index of pulmonary function in different severities of bronchial asthma morningsickness at different stages.Methods The levels of serum TIgE and serum ECP in 53 cases of bronchial asthma morningsickness at acute episode stage and remission stage were determined.The pulmonary function was detected at the same time.20 cases of healthy normal children were chosen as control.Results The level of serum ECP in bronchial asthma morningsickness at acute episode stage was obviously higher than that at remission stage and that in control group(P<0.01).There was significant difference on the level of serum ECP among different severities of asthma morningsickness at acute episode stage,but there was no difference at remission stage.The level of serum TIgE at both acute episode stage and remission stage was higher than that in control group(P<0.01),but there was no significant difference between acute episode stage and remission stage and among different severities.The level of serum ECP was negatively correlated with the tested value and percentage of pulmonary function FEV1(FEV1%pre), the ratio of one second forced expiration volume and forced vital capacity(FEV1/FVC).There was no correlation between serum TIgE and pulmonary function.Conclusion There is direct relation among the level of serum ECP heightened and pulmonary function decreased,respiratory tract responsiveness heightened and asthma episode.The level of serum ECP can exactly reflect the active state of eosinophilic granulocyte and the change of air passage inflammation in morningsickness,and it is a reliable indicator for diagnosis,pathogenetic condition judgement and guiding therapy.
