胆囊结石时胆囊粘膜的病理改变和癌胚抗原的分布特征

为了探讨结石对胆囊粘膜的病理影响，观察１０７例胆囊结石患者结石的性状、大小．对新鲜胆囊粘膜上皮用单克隆抗体癌胚抗原（ＣＥＡ）标记，观察ＣＥＡ的存在、程度、分布，分析了胆囊粘膜的病理改变情况。结果表明：随着年龄的增加．胆囊粘膜的病理性损害逐渐加重．结石的物理性状在良性与恶性病变组间有相关关系；结石的大小．特别是结石直径≥１０ｍｍ时．不仅使这种病理演变过程更加明显．而且引起胆囊癌变的危险因素也显著增加。通过对本组１０７例胆囊粘膜ＣＥＡ标记．我们发现ＣＥＡ膜型阳性难以区分胆囊粘膜的病变程度．但ＣＥＡ浆型阳性对胆囊癌细胞及癌前病变具有特征性意义。

Pathologic changes and carcinoembryonic antigen(CEA) distribution characteristics in cholelithiasic gallbladder mucosa

consecutive operative specimens of cholelithiasic gallbladder were studied for the phisico-chemical characters of the stones and for the presence and distribution of CEA in fresh mucosal epithelium using monoclonal antibody techenique, in order to find pathologic alteration in the gallbledder mucosa in relation to the ptesence of gallstones. We found that the anthologic impelment of the gallbladder mucosa increased perallely with the patients ase and the physical characters of the stones might have some correlations between benign and malignant groups. A remarkable difference of stone size eds,ted between gallbladder mucosal proliferation group(Group I)and other groups(P<0. 05),especially in stones of which the diameter was larger than 10 mm. The latter may aggravate the pathologic process and enhance the risk of malisnancy of the gallbladder. Through monoclonal symbolization of CEA on the 107 specimens of cholecystic mucosa we found that CEA of membrane type was unable to define the pathologic state of gallbladder mucosa, while CEA of plasmatic type might possess a characteristic sisnificance for tedancy of gnllbladder cancer or precancerous state.
