帕罗西汀对心房颤动患者焦虑抑郁情绪生活质量及临床疗效的作用

目的 探讨心房颤动(Atrial Fibrillation,Af)患者合并焦虑、抑郁情绪的发生率,观察帕罗西汀对Af患者合并焦虑、抑郁情绪的临床疗效、帕罗西汀治疗后Af患者Af的转归.方法 采用汉密顿焦虑量表(HAMA)和汉密顿抑郁量表(HAMD-24)评定Af患者的焦虑、抑郁情绪;采用健康状况调查问卷(SF-36)评定患者的生活质量.将筛查出的合并焦虑、抑郁情绪的Af患者采用随机信封的方法 分为盐酸帕罗西汀治疗组和谷维素对照组,分别观察治疗前、后2组患者HAMA、HAMD-24及健康状况的变化以及Af的临床疗效.结果 Af患者合并焦虑、抑郁情绪的发生率分别为32.9%、37.2%;帕罗西汀治疗组治疗后HAMA、HAMD-24评分分别为(13.19±5.88)分、(20.57±6.24)分,较对照组[HAMA评分(19.84±5.02)分、HAMD-24评分(29.75±8.63)分]明显降低(P<0.01),SF-36生活质量评分提高(P<0.01);阵发性Af、持续性Af的治疗有效率分别为88.8%、75.O%.结论 Af患者合并焦虑、抑郁情绪的发生率较高,生活质量差;盐酸帕罗西汀治疗能够改善Af患者的焦虑、抑郁情绪,改善Af患者的生活质量,并有助于控制阵发性Af及持续性Af的症状.

The effects of paroxetine hydrochloride on quality of life, clinical result of atrial fibrillation patients with anxiety or depression

Objective To investigate the prevalence rate of anxiety or depression in patients with atrial fibriUation (Af) disease and the result of influence of paroxetine hydrochloride,including the changes of the health situation and the prognosis of different kinds of Af.Methods Totally 140 patients who underwent Af were assessed with HAMA and HAMD-24 before and after the treatment.65 patients with anxiety and depression were evaluated with 36-Health survey questionnaire (SF-36) and divided into two groups at random,one of which was prescribed paroxetine hydrochloride 20～60mg once a day on the basis of routine cardiac drug therapy for 3 months and the other oryzarol tablets.Results The prevalence rate of anxiety and depression in patients with Af was 32.9% ,37.2% respectively.The exponent of HAMA and HAMD-24 in paroxetine hydrochloride group after treatment(HAMA 19.84±5.02,HAMD-24 29.75±8.63) was lower than that of before (HAMA 22.35±6.94,HAMD-24 33.79±10.01) ,and lower than that of oryzanol tablets.The scores of SF-36 was higher and the attack frequency of Af after treatment of paroxetine hydrochloride was lower than that of before.The curative rate of paroxetine hydrochloride on paroxysmal Af and permanent Af was 88.8% ,75.0%.Conclusion The prevalence rate of anxiety and depression in patients with Af is high,Paroxetine hydrochloride can relieve the symptoms of anxiety and depression in Af patients and control symptoms of paroxysmal Af and permanent Af.
