氯氮平与阿立哌唑对精神分裂症患者代谢综合征的影响

目的研究氯氮平与阿立哌唑对精神分裂症患者代谢综合征的影响。方法将60例精神分裂症患者随机分为2组,分别给予氯氮平、阿利哌唑治疗,测量治疗前及治疗后2、4、8、16、24周体重指数(BMI)、腰围、血糖、血脂等指标的变化。结果治疗24周后,氯氮平组、阿立哌唑组的代谢综合征的发病率分别为43.33%、13.33%,两组比较差异有统计学意义(P<0.05);治疗8周后,两组BMI、腰围、FPG、HbA1c、TG均升高,治疗24周后达到高峰期;氯氮平组治疗8周后BMI、腰围、FPG、HbA1c、TG水平均高于阿立哌唑组,两组比较差异有统计学意义(P<0.05)。结论阿立哌唑对精神分裂症患者代谢指标的影响较氯氮平小,阿立哌唑是一种安全性更高的非经典抗精神病药物。

Effect of clozapine and aripiprazole on metabolic syndrome in patients with schizo-phrenia

Objective To study the effect of clozapine and aripiprazole on metabolic syndrome in patients with schizophrenia. Methods 60 patients with schizophrenia were randomly divided into two groups, and the patients were treated with clozapine and aripiprazole respectively. The BMI, waist circumference, blood glucose, blood lipid changes were measured before treatment and at 2 weeks,4 weeks,8 weeks, 16 weeks and 24 weeks after treatment. Results After 24 weeks of treatment,the incidence of metabolic syndrome in clozapine group （43.33%） was higher than that of aripiprazole group （3.33 % ）, there was significant difference between the two groups （P 〈 0. 05 ） ; after 8 weeks of treatment, the BMI, waist circumference, FPG, HbA1 c and TG of the two groups increased, and the indexes reached the peak at 24 weeks after treatment; the BMI, waist circumference, FPG, HbAlc and TG levels in clozapine group were higher than those of aripiprazole group, there were significant differences between the two groups （ P 〈 0. 05 ）. Conclu- sion Compared with clozapine, aripiprazole is a non-classical antipsychotics with smaller impact on metabolic parame- ters in patients with schizophrenia.
