彩色多普勒超声诊断小儿面颈部淋巴管瘤

摘    要:目的 本研究旨在总结小儿面颈部淋巴管瘤的声像图特征和血流动力学变化特点,探讨B型超声结合CDFI在提高其诊断准确性方面的应用价值。方法 采用彩色多普勒超声仪观察56例患儿的病变部位、大小、形态及内部回声结构,同时用CDFI观察包块周边和内部的血供,并在注射治疗后随访超声图像变化。结果 56例中超声诊断与病理诊断符合者54例(%.4%)。小儿面颈部淋巴管瘤声像图典型特征为面颈部皮下软组织内边界清晰或略欠清晰的囊性包块,其内有强回声条状分隔,CDFI显示分隔上有短条状血流信号。注射治疗后瘤体消退明显。结论 CDFI能早期诊断淋巴管瘤,为临床选择手术途径、注射药物治疗及评价疗效提供可靠的图像依据,具有较好的临床实用价值。

Abstract:Objective To analyse the sonographic characteristics and the hemodynamic changes of facial and cervical lymphangiomas. Methods Color Doppler flow imageing(CDFI) was performed in 56 cases to assess the extension, location and internal echo. CDFI was also used to provide information about vascularity of the lesion and the tissues surround it. Clinical follow-up and ultrasonography examinations were performed to evaluate regression of the lesion.Results The accuracy rate of US was 96.4%.The typical sonogram of lymphangioma was well-defined cystls with several thin septa.CDFI showed mottled blood supply in the septa.Regression was excellent in 8 patients who were treated with percutaneous sclerotherapy.Conclusion CDFI can help establish the stage and structure of the lesion.So the unique characteristics of facial and cervical lymphangioma can be used in clinical practice for the diagnosis and differential diagnosis.
