加味四妙勇安汤联合前列地尔注射液治疗下肢动脉闭塞90例临床研究

目的:观察加味四妙勇安汤联合前列地尔注射液治疗下肢动脉闭塞疗效。方法:将90例下肢动脉闭塞症患者随机分为治疗组和对照组,各45例。治疗组给予加味四妙勇安汤联合前列地尔注射液治疗,加味四妙勇安汤每日1剂,水煎服,连用3周;对照组给予前列地尔注射液治疗。其它情况处理,两组同样对待。前列地尔注射液2 m L+生理盐水10 m L缓慢静注,连续治疗两周为1个疗程,每20天1个疗程,共治疗4个疗程,其他治疗两组相同。观察两组患者治疗结束后临床疗效、血浆血脂及两组病人治疗前后症状变化。结果:治疗组患者总有效率为97.8%,高于对照组的88.9%,比较两组患者的总有效率,经检验后得出差异有统计学意义(χ2=4.94,P=0.026)。两组患者治疗结束后的总胆固醇、低密度脂蛋白均较治疗前有所降低,而高密度脂蛋白较治疗前则有所升高,与对照组相比差异具有统计学意义(P<0.05)。两组患者的ABI值也较治疗前有所升高,但治疗组ABI升高值明显大于对照组,差异具有统计学意义(P<0.03)。两组患者症状体征积分比较,差异具有统计学意义(P<0.001)。结论:加味四妙勇安汤联合前列地尔注射液治疗下肢动脉闭塞症临床疗效肯定,可能通过降低患者的血浆血脂而起作用。

Clinical Efficacy of Jiawei Simiao Yong＇an Decoction and Alprostadil Injection for 90 Patients with Lower Extremity Arterial Occlusion

Objective ：To observe the clinical efficacy of Alprostadil Injection and Jiawei Simiao Yong＇an Decoction in treating the lower extremity arterial occlusion and the effects on blood lipid. Methods：90 cases of the lower extremity arterial occlusion were randomly divided into treatment group （45 cases） and control group （45 cases）. The treatment group was given Jiawei Simi- ao Yong＇an Decoction one dose a day and Alprostadil Injection. The control group was given the Alprostadil Injection. Alprostadil Injection 2 mL and physiological saline 10 mL were given by slow intravenous injection. Continuous 2 weeks treatment was one course and 20 d was a course, totally for 4 courses. The other treatments were the same in the two groups. We observe the effect rate, serum lipid levels and symptoms before and after treatment in the two groups. Results ：The total effective rate of the treatment group was 97.8 % and that of the control group was 88.9 %. There was significant difference （ P 〈 0.05 ）. The total cholesterol and the low density lipoprotein were lower than those before treatment in both two groups and the high density lipoprotein was in- creased than that before treatment in the two groups. Compared with the control group, the difference was statistically significant （P 〈 0. 05 ）. ABI values of the two groups were also increased. The treatment group＇s ABI values were significantly greater than that in the control group and the difference was statistically significant （P 〈 0.03 ）. Two group＇symptoms had a statistically signifi- cant difference （P 〈 0. 001 ）. Conclusion：Alprostadil Injection and Jiawei Simiao Yon＇gan Decoction treatment of lower extremity arterial occlusion has affirmed clinical curative effect, which may act by reducing plasma lipids in patients.
