B超定位在体外冲击波碎石治疗输尿管结石中的应用研究

摘    要:目的探讨B超定位在体外冲击波碎石治疗输尿管结石中的应用研究。方法我院2001年6月~2009年6月体外冲击波碎石治疗输尿管结石采用B超定位84例,根据输尿管的部位分为输尿管上段结石28例,输尿管中段结石12例,输尿管下段结石44例三组,分别观察在B超定位下三组体外冲击波碎石治疗的成功率及复震情况。结果本组84例输尿管结石在B超定位行ESWL治疗后的成功率为95.24%(80/84)。一次性碎石成功69例,占82.14%;多次复震16例,最多达4次,平均2.4次,复震率为19.05%。B超定位不同分段输尿管结石的治疗的成功率及复震情况比较差异无统计学意义。下段结石所占比例比较高,达52.38%(44/84),其发生率明显高于上段及中段结石,且B超显示其平均直径为0.5cm,最大直径0.6cm。结论体外冲击波碎石时通过B超进行定位,可提供任何切面的适时超声图像,经过实时数字化处理形成高分辨率的切面声像图,无论结石大小或密度高低,均可在声像图上以亮点形式显像,提高了碎石的成功率,值得进一步推广与应用。

Abstract:Objective B-Uiteasonic Localization in extracorporeal shock wave lithotripsy for ureteral stones in the application.Methods In our hospital from June 2001 to June 2009 by B-Uiteasonic Localization in extracorporeal shock wave lithotripsy of ureteral stones in 84 patients treated,according to the site of ureteral ureteral stones were divided into 28 cases of middle ureteral stones in 12 cases,ureteral calculi three groups of 44 patients were under observation in the B-locate three ESWL success rates of treatment and rehabilitation earthquake situation.Results The group of 84 cases of ureteral calculi in the B-orientation lines after ESWL treatment success rate was 95.24%(80/84).69 cases of successful single stone,accounting for 82.14%;many complex earthquake in 16 cases,up to 4 times the average 2.4 times,complex shock was 19.05%.All patients had hematuria (gross or microscopic),but are no special handling.B-positioning treatment of ureteral stones in different sub-success rate and the situation is more complex shock, Statistically,the difference was not statistically significant.The bottom section of a relatively high proportion of stones,up to 52.38%(44/84),its incidence was significantly higher than the upper and middle stone,and the B-shows the average diameter of 0.5cm, diameter 0.6cm.Conclusion The extracorporeal shock wave lithotripsy, through the B-Uiteasonic image , to provide ultrasound images of any aspect of timely,real-time digital processing after the formation of high-resolution ultrasonography section, regardless of the level of stone size or density,can be found in ultrasonography on to highlight the form of imaging to improve the success rate of stone,worthy of further promotion and application.
