针药并治干预卒中后抑郁的临床观察

目的观察针药并治干预卒中后抑郁(PSD)的疗效,并初步探讨其机制。方法60例卒中后抑郁患者随机分为针药并治组和万拉法新组,在给予相同脑卒中后治疗方案的基础上,分别给予针刺加中药治疗和万拉法新治疗,于治疗前、治疗6周后采用相应量表评价患者抑郁程度、神经功能缺损、不良反应;在治疗前、治疗6周后取脑脊液检测去甲肾上腺素(NE)、5-羟色胺(5-HT)浓度。结果2组均对PSD治疗有效、能促进神经功能缺损恢复,组间比较无显著性差异;万拉法新组不良反应较多;2组脑脊液NE、5-HT浓度变化无显著性差异。结论在脑卒中后治疗基上予针药用治疗PSD效果优于应用万拉法新。

Clinical observation on treatment of post stroke depression by acupuncture combined with drugs

Objective It is to observe the effect of post stroke depression treated by acupuncture combined with Chinese herbal compound,and to explore its mechanism.Methods 60 patients with post stroke depression were randomly divided into two groups,the treatment group was given acupuncture and Chinese herbal compound,and the control group was given venlafaxine.The degree of depression,neurologic impairment and adverse effect were evaluated by the corresponding rating scales in pre-treatment and six weeks after treatment;the concentration of noradrenergic(NE) and 5-hydroxytryptamine(5-HT) in cerebrospinal fluid were detected by high performance liquid chromatogram in pre-treatment and six weeks after treatment.Results Both treatments could cure PSD,and were help for rehabilitation,there were no significant difference between both groups;and the treatment group had low adverse effect rate than the control group;the concentration of NE and 5-HT was similar between both groups.Conclusion Acupuncture combined with drugs is better than venlafaxine in the treatment of PSD.
