双侧髋关节同期置换比较优势的临床研究

目的比较双侧髋关节同期置换与分期置换的临床疗效,得出同期置换中存在的优势。方法回顾性研究自2005年7月至2009年7月期间我科行双侧髋关节置换术的54例患者分为2组：同期组33例（66髋）,分期组21例（42髋）。比较2组住院费用,总住院时间,总出血量,总输血量,术前和术后Harris评分及手术后并发症等方面的差异（含术后关节活动度）。结果 2组在住院费用、总住院时间、总手术时间、总出血量方面差异有显著性（P〈0.05）,分期置换均要大于同期置换,在术后Harris评分,总输血量方面差异无显著性（P〉0.05）。结论同期双侧髋关节置换具有明显的比较优势,值得推广。

The Clinical Study of Comparative Advantage on One-stage Simultaneous Total Hip Arthroplasty in Patients with Bilateral Hip Disease

Objective To compare the clinical efficacies of one-stage and two-stage total hip arthroplasty in patients with bilateral hip disease,obtain the comparative advantage in the one-stage total hip arthroplasty.Methods Retrospective study 54 patients who received bilateral hip arthroplasty in our department from July 2005 to July 2009,and divide them into two groups：group one-stage 33 patients（66 hips）who given one-stage operation,group two-stage 21 patients（42 hips） who given two-stage operation.The costs of hospitalization,total times of hospitalization,times of operative duration,total amounts of bleeding,total volumes of blood transfusion,preoperative and postoperative Harris scores,and surgical complications of the two groups were compared（including the active scope of hip joint）.Results The costs of hospitalization,total times of hospitalization,total operative times of duration,total amounts of bleeding and were of significant differences（P〈0.05）,two-stage is all more than one-stage,but the preoperative and postoperative Harris scores,total volumes of blood transfusion between the two groups showed no significantly statistical difference（P〈0.05）.Conclusion The one-stage total hip arthroplasty has obvious comparative advantage,which should be popularized.
