20例原发性黏膜恶性黑色素瘤临床病理特征

目的探讨黏膜黑色素瘤的临床病理特征.方法对20例黏膜黑色素瘤进行光镜观察,并对部分病例作组化及免疫组化染色.结果 20例中,小上皮细胞型6例、大上皮细胞型5例、未分化癌样型4例、纤维肉瘤样型3例、淋巴肉瘤样型与血管外皮瘤样型各1例.光镜形态与皮肤恶性黑色素瘤类似.免疫表型S-100阳性者18例,阳性率为90%;HMB45阳性者17例,阳性率为85%.结论黏膜恶性黑色素瘤的临床病理特点及免疫表型与皮肤黑色素瘤无明显区别.黏膜上皮的黑色素细胞与黑痣是黏膜黑色素瘤的发生基础.

CLINICOPATHOLOGICAL ANALYSIS OF 20 CASES OF PRIMARY MELANOMA OF MUCOUS MEMBRANES

Objective To explore the clinicopathological characteristics of primary melanoma of mucous membranes. Methods 20 cases of melancoma of mucous membranes were observed under optical microscope and some of them underwent histochemical and immunohistochemical staining. Results These cases were classified into 6 cases of minor epithelioma,5 case.s of major epithelioma,4 cases of undifferentiated tumors,3 cases of fibrosarcoma, case of lymphosarcoma and 1 case of hemangiopericytoma. These tumors were similar to skin melancoma in appearance. The positive rates of immunological expressions 5-10 in 18 cases and HMBA5 in 17 cases were 90% and 85 % respectively. Conclusion No significant differences were found in the clinicopathological characteristics or immunological expressions between melanoma of mucous membrane and skin melanoma. Melanocytes and melanotic nevi form the basis for melanoma of mucous membrane.
