不同类型脂肪酸饮食对肠促胰岛素及胰岛功能的影响

摘    要:目的:探讨不同类型脂肪酸饮食对糖耐量正常受试者肠促胰岛素及胰岛功能的影响。方法选择2011年6月至2015年6月60例体检正常的受检者,按照数字列表法随机分为两组各30例,食用标准饮食洗脱1周,其中给予高饱和脂肪酸饮食(HSF)组为HSF组,给予高不饱和脂肪酸饮食(HMF)组为HMF组,比较两组受检者进餐前(T0)、进餐后60 min (T1)、120 min (T2)、180 min (T3)、240 min (T4)的血糖、胰岛素(FIns)、抑胃多肽(GIP)、胰高血糖素样肽-1(GLP-1)、胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)以及胰岛素早时相分泌功能指数(ΔI30/ΔG30)。结果①两组受检者进餐后的FPG、GIP、GLP-1均呈升高后缓慢下降走势,T1、T2、T3、T4各时间段比较差异均有统计学意义(P<0.05),HMF组各时间段GIP、GLP-1均高于HSF组,差异均有统计学意义(P<0.05);②两组受检者进餐后FIns均呈升高后缓慢下降走势,各时间段比较差异均有统计学意义(P<0.05),HMF组T1、T2、T3、T4各时间段FIns均低于HSF组,差异均有统计学意义(P<0.05);③HSF组与HMF组受检者ΔI30/ΔG30分别为(47.08±10.32)、(42.55±9.83),差异无统计学意义(P>0.05),HSF组AUCins 240 min/AUCglu 240 min为(3.65±0.56),明显高于HMF组的(2.86±0.46),差异有统计学意义(P<0.05)。结论高不饱和脂肪酸饮食促进GIP、GLP-1分泌作用最明显,HSF可促进更多胰岛素的分泌,胰岛β细胞功能指数也明显增高。

Effect of different types of fatty acid diets on incretin and islet function
Abstract:Objective To investigate the effect of different types of fatty acid diets on incretin and islet func-tion. Methods A total of 60 cases of physical examination normal volunteers in our hospital from June 2011 to June 2015 were enrolled in this study and randomly divided into two groups according to the digital list method, with 30 cases in each group. After a standard diet eluting for one week, one group was given diets high in saturated fatty acids (HSF), as the HSF group, and another group was given high monounsaturated fat acid diet (HMF), as the HMF group. At 0 min (T0), 60 min (T1), 120 min (T2), 180 min (T3), 240 min (T4) following meal intake, the plasma concentrations of glu-cose, insulin (FIns), gastric inhibitory polypeptide (GIP), glucagon-like peptide-1 (GLP-1) and pancreaticβ cell func-tion index (HOMA-β), insulin resistance index (HOMA-IR) and insulin early phase secretion function index (ΔI30/ΔG30) before in two groups were compared. Results ①After meals, FPG, GIP and GLP-1 in the two groups showed an slow decline trend after increase, and the differences at T1, T2, T3, T4 were statistically significant (P<0.05);GIP, GLP-1 at each time points in HMF group were significantly higher than those in HSF group (P<0.05).②After meals, FIns in the two groups showed an slow decline trend after increase, and the differences at each time points were statistically sig-nificant (P<0.05);FIns at T1, T2, T3, T4 time points in HMF group were significantly lower than those in HSF group (P<0.05).③There was no significant difference between HSF group (47.08±10.32) and HMF group (42.55± 9.83) inΔI30/ΔG30 (P>0.05);There was significant difference between HSF group (3.65±0.56) and HMF group (2.86±0.46) in AUCins 240 min/AUCglu 240 min (P<0.05). Conclusion Highly unsaturated fatty acid diets have the most significant promo-tion on GIP, GLP-1 secretion. HSF can promote more secretion of insulin, and significantly increase the islet β cell function index.
