孕妇血清GH、IGF-1、IGFBP3水平的变化及其与新生儿生长参数的相关性研究

目的 探讨生长激素(GH)、胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP3)与妊娠期糖尿病(GDM)的关系及其对新生儿生长参数的影响.方法 选择2014年11月至2015年7月在南宁市妇幼保健院住院分娩、孕期检查为妊娠期糖尿病并且孕期通过医学营养治疗血糖控制良好的孕妇42例,作为GDM组,另随机选取同期糖耐量试验正常的孕妇40例,作为对照组.采用电化学发光法测定两组孕妇血清GH、IGF-1、IGFBP3水平.孕妇分娩后测量新生儿体质量、身长、头围、肩围、胸围、胎盘重量等生长参数.比较两组孕妇血清GH、IGF-1、IGFBP3的差异以及与上述新生儿生长参数的相关性.结果 GDM组孕妇的IGF-1和IGFBP3分别为(343.3±101.9)ng/mL和(5.5±1.1)μg/mL,均高于对照组的(267.5±84.9)ng/mL和(4.8±0.9)μg/mL,差异均有统计学意义(P<0.05);两组孕妇的GH、新生儿体质量、身长、头围、肩围、胸围、胎盘重量比较差异均无统计学意义(P>0.05).结论 孕妇外周血GH、IGF-1、IGFBP3水平与新生儿体质量、身长、头围、肩围、胸围、胎盘重量等生长参数无显著相关性,但是IGF-1、IGFBP3与GDM的发生相关,可作为一种预测GDM的潜在指标.

Study on correlation between the change of GH,IGF-1, IGFBP3 of gestational diabetes mellitus and the growth parameters of newborn

Objective To investigate the relationship between the growth hormone (GH), insulin like growth fac-tor-1 (LGF1), insulin like growth factor binding protein 3 (IGFBP3) and the gestational diabetes mellitus (GDM) and futher to know their effect to the growth parameters of newborn. Methods Forty-two pregnant women diagnosed as gesta-tional diabetes mellitus were chosen as the GDM group after their blood glucose had been controlled by medical nutrition therapy and delivered in Nanning Maternal and Child Health Care Hospital from November 2014 to July 2015, and forty pregnant women whose glucose tolerance test results were normal were randomly selected as the control group. Electro-chemical luminescence method was used to test the level of GH, IGF-1, IGFBP3 of the two groups, and the weight, body length, head circumference, shoulder circumference, chest circumference, placental weight of their newborns were also measured. And then the differences of the two groups were compared. Results The IGF-1 and IGFBP3 of the GDM group were separately (343.3 ± 101.9) ng/mL and (5.5 ± 1.1)μg/mL, which were significantly higher than the results of the control group [(267.5 ± 84.9) ng/mL, (4.8 ± 0.9)μg/mL, P<0.05]. But no significant difference was found of pregnant wom-an's growth hormone, newborn's weight, body length, head circumference, shoulder circumference, chest circumference and placental weight between the two groups (P>0.05). Conclusion There was no significant correlation between the levels of GH, IGF-1, IGFBP3 of the pregnant women's peripheral blood and the growth parameters of newborns, such as weight, body length, head circumference, shoulder circumference, chest circumference, placental weight, but the the levels of IGF-1 and IGFBP3 were related to the incidence of GDM, which could be used as potential indicators for predicting GDM.
