经阴道彩色多普勒超声诊断剖宫产术后子宫切口妊娠的临床价值

目的：探讨经阴道彩色多普勒超声对剖宫产术后子宫切口妊娠的诊断价值。方法将2012年1月至2014年6月于我院妇产科接受治疗的100例子宫切口妊娠孕妇纳入研究，所有患者均经阴道彩色多普勒超声检查，重点对血流影像图及超声分型检查进行回顾性分析。结果100例剖宫产术后瘢痕子宫切口妊娠患者中，94例经阴道彩色多普勒超声诊断与病理诊断结果一致，诊断准确率为94%，超声图像特征包括早早孕型9例，胚囊型43例，不均质包块型42例。6例患者诊断错误，误诊率为6%。100例患者中37例患者接受手术治疗，63例接受保守治疗，所有患者均安全出院。结论经阴道彩色多普勒超声诊断剖宫产术后子宫切口妊娠有效性较高，值得临床推广应用。

Clinical study of transvaginal color Doppler ultrasound in the diagnosis of uterine incision pregnancy after cesarean surgery

Objective To discuss the value of transvaginal color Doppler ultrasound in the diagnosis of uterine incision pregnancy after cesarean surgery, and provide theoretical basis for clinical work. Methods One hundred preg-nant women with uterine incision pregnancy treated from January 2012 to June 2014 in Department of Obstetrics and Gynecology of our hospital were selected as the study objects. All patients were detected by transvaginal color Doppler ultrasound examination,and the blood flow imaging and ultrasound typing examination were retrospectively analyzed. Results Among the 100 women, the transvaginal color Doppler ultrasound results of 94 pregnant women with cesarean scar pregnancy was consistent with clinical or pathological diagnosis results, and the diagnostic accuracy was 94%. Ultra-sonic image features showed 9 cases of early pregnancy, 43 cases of embryo, 42 cases of heterogeneous mass. Six cases were misdiagnosed, and the misdiagnosis rate was 6%. Thirty-seven of the 100 patients received surgical treatment, and 63 patients received conservative treatment, with all patients discharged safely. Conclusion The efficacy of transvagi-nal color Doppler ultrasound in the diagnosis of uterine incision pregnancy after cesarean surgery is relatively high, and it is worth widely promoting in clinic.
