| 摘 要： | 目的:探讨全直肠系膜切除(TME)及吻合器在低位直肠癌根治术中的应用。方法:对2003年6月～2006年4月收治的32例低位直肠癌术中应用吻合器保留肛门的病例资料进行回顾性分析。手术方法按照全直肠系膜切除(TME)原则,切除近端肠管至少15cm、肿瘤下缘远端2～3cm的直肠,应用吻合器在骶前行结-直肠端端吻合术以保留肛门。结果:术后无吻合口瘘发生,患者肛门便排及控制排便功能半年后接近正常。全组随访1.5～2年,出现1例复发,再次行Miles氏术。结论:TME技术应用吻合器解决了盆腔深部进行低位吻合的困难,具有操作简单、方便、安全等优点,能提高患者的生存质量。
