胃缩口吻合术的临床应用

目的评价胃缩口吻合术的临床效果。方法回顾性分析119例行胃部分切除术应用胃缩口吻合术重建消化道的临床资料,其中行远端胃大部切除胃十二指肠吻合36例,远端胃大部切除RouxenY吻合术52例,远端胃大部切除胃空肠端侧吻合21例,近端胃大部切除食管胃吻合8例,胰十二指肠切除术2例。结果本组平均吻合时间25(20～35)min,无手术死亡病例,无一例吻合口漏。术后发生近期并发症15例(12.6%),其中切口感染5例,肺部感染4例,粘连性肠梗阻4例,反流性胃炎2例。无腹泻、食欲不振等症状,无倾倒综合征。随访101例,平均随访3(1～4)年,未发现吻合口狭窄。结论胃缩口吻合术重建消化道简单易行,减少了术后并发症,提高了患者术后生活质量。

Clinical Application of Gastric Deflated Mouth Anastomosis

Objective To evaluate the clinical outcome of gastric deflated mouths anastomosis. Methods The clinical data of 119 patients underwent various gastrectomy with gastric deflated mouths anastomosis were analyzed retrospectively. Results The mean anastomosis duration was 25 minutes. No death and anastomotic stoma leakage occurred. Postoperative complications occurred in 15 patients (12.6%), including incision infection in 5 cases, pulmonary infection in 4 cases, adhesiveileus in 4 cases, and reflux gastritis in 2 cases. There was no abdominal distention, anorexia, and dumping syndrome in these patients. No anastomotic stricture occurred in 101 cases with 1 to 4 year follow-up. Conclusion The surgical procedure is simple and can decrease the postoperative complications. The life qualities of these patients after operation were satisfied.
