痰涂片在下呼吸道感染中的应用价值探讨

目的:研究痰涂片在下呼吸道感染诊断中的应用价值,为下呼吸道感染提供更准确的诊断依据。方法:收集860份痰标本,分别做痰涂片和痰培养,用痰涂片筛选合格痰标本,并对潜在病原菌用Vitek 2 COmpact系统及配套鉴定卡进行鉴定。结果:860份痰标本中合格痰标本380份,合格率为44.2%。380份合格痰标本中痰培养阳性率为57.4%;480份不合格痰标本痰培养阳性率为5.6%,两者之间差异有统计学意义(P<0.01)。在380份合格痰标本中,涂片与培养结果总符合率为76.3%。结论:合格痰标本是痰培养的必要条件,合格标本中痰涂片与培养结果符合率较高。痰涂片不仅能筛选合格痰标本,并且在痰培养中区分病原菌与污染菌起着重要作用。重视痰涂片能有效降低痰培养的假阳性率和假阴性率。

To dicuss the application value of sputum smear in lower respiratory tract infection

Objective To study the application value of sputum smear in lower respiratory tract infection,to provide more accurate diagnostic basis for lower respiratory tract infection.Methods 860 sputum specimens were done sputum smear and sputum culture.Sputum smear was used as screening qualified sputum specimens,Vitek 2 Compact system and supporting identification cards were used to identify potential pathogens. Results There are 380 qualified sputum specimens in 860 sputum specimens,the percent of pass is 44.2%.The sputum culture positive rate of 380 qualified sputum specimens is 57.4%;the sputum culture positive rate of 480 non-qualified sputum specimens is 5.6%.There is significant difference（P＆lt;0.01）.In 380 sputum specimens,the total Coincidence rate of smear and culture Results is 76.3%.Conclusion Qualified sputum specimen is the necessary Condition of sputum culture.The total Coincidence rate of smear and culture Results is very high.Sputum smear can screen qualified sputum specimens,and it also plays an important role in distinguishing the pathogen and the polluted bacteria.Paying more attention to the sputum smear can effectively reduce false positive and false negative rate of the sputum culture.
