高分辨率CT在肺间质纤维化的诊断及应用

摘    要:目的:探讨高分辨率CT(HRCT)在肺间质纤维化中的诊断价值及临床应用。方法:随机选择2010-01~2011-11间84例肺间质纤维化患者作为研究对象,行直接数字化X线摄影系统(CR)、电子计算机X线断层扫描技术(CT)进行检查。在此基础上对主动脉弓、支气管分叉、隆突下、右肺门、肺底膈上等区域的局灶性或弥漫性病变区行高分辨率CT扫描。对各项影像学检查结果进行分析和比较。结果:高分辨率CT检查结果显示双肺或单侧肺的基底部可见网织状阴影、磨玻璃样改变,密度较低,在阴影区内可见肺血管纹理。54例患者可见肺小叶间隔增厚,有43例患者可见胸膜增厚。28例患者肺部出现蜂窝状改变,小叶结构模糊。HRCT检查均被诊断为弥漫性间质性肺部纤维化。三种影像学检查方法对弥漫性肺间质性纤维化的检出率差异有统计学意义。结论:高分辨率CT在肺间质纤维化的诊断和鉴别诊断上具有较好的应用价值。

Abstract:Objective:To investigate the diagnosis value and clinical application of pulmonary fibrosis with High resolution CT(HRCT).Methods:This study randomly selected 84 cases of pulmonary interstitial fibrosis patients in our hospital from 2009 January to 2011 January as the research objects,and examined them with direct digital radiography system(CR) and computed tomography(CT) examination technology.The focal or diffuse lesion area of aortic arch,bronchial bifurcation,carina,right hilar lung bottom diaphragm and upper regions were scanned with high resolution CT.The imaging examination results were analyzed and compared.Results:High resolution CT examination showed on the base of bilateral pulmonary or unilateral pulmonary reticulocyte shape shadow,ground glass appearance changes could be seen,and the density was lower.In the shadow zone the pulmonary vascular texture could be seen.In 54 patients lung interlobular septal thickening could be seen,in 43 cases pleural thickening could be seen.In 28 patients lung honeycombing,and fuzzy lobular structure could be seen.HRCT examination found all these patients diffuse interstitial pulmonary fibrosis.The detection rate of three kinds of imaging methods for diffuse pulmonary interstitial fibrosis had a significant difference.Conclusions:high resolution had good application value in the diagnosis and differential diagnosis of pulmonary interstitial fibrosis.
