| 摘 要： | 目的:分析极低出生体重儿(VLBW I)并发症的发生情况及死亡原因,并对婴幼儿时期生长发育进行随访。方法:对我市二医院及市妇保院新生儿病房2002年7月~2006年6月收治的132例极低出生体重儿进行回顾性分析,并对资料齐全的38例进行随访监测,测定其发育商(DQ)及身体生长发育值。结果:所有住院患儿均有一种或一种以上并发症,主要有低体温82例(62.1%)、呼吸暂停80例(60.6%)、呼吸窘迫综合征(RDS)37例(28.0%)、贫血88例(66.7%)、新生儿慢性肺部疾病(CLD)16例(12.1%)、代谢紊乱64例(48.5%)、脑室内出血(IVH)23例(17.4%)。死亡原因主要有RDS、IVH、感染、肺出血。体重越低,并发症的发生率越高,死亡率越高。VLBW I在婴幼儿时期身高、体重指标在正常范围内的约占2/3,DQ在中下水平及以下者占36.8%。结论:提高VLBW I存活率、降低致残率必须重视产前检查,加强围产期保健;密切监护,及时处理并发症;加强其婴幼儿期的保健工作,积极指导早期教育。
