经脐单孔腹腔镜与传统腹腔镜胆囊切除术临床对照研究

目的 对比经脐单孔腹腔镜与传统腹腔镜胆囊切除术在临床上的应用效果.方法 胆囊炎患者60例根据手术方法的不同分为治疗组与对照组各30例,对照组采用传统腹腔镜胆囊切除术,治疗组采用经脐单孔腹腔镜胆囊切除术.结果 两组患者全部治愈,治疗组的术后住院时间与总住院花费明显少于对照组（P〈0.05）.治疗组术后切口感染、胆道损伤、背部疼痛、切口出血等总体并发症发生率明显少于对照组（P〈0.05）.两组治疗前IgG含量对比无明显差异,治疗后都有明显下降（P〈0.05）,同时组间对比差异明显（P〈0.05）.结论 经脐单孔腹腔镜胆囊切除术应用于胆囊炎的治疗更加有利于降低术后并发症发生率,有利于患者的恢复,同时对于体液的免疫功能影响小.

The controlled clinical study of umbilical single-port laparoscopic and conventional laparoscopic cholecystectomy

Objective To compare clinical application effects of umbilical single-port laparoscopic and conventional laparoscopic chole- cystectomy. Methods 60 patients with cholecystitis accorded to different surgical .methods were equally divided into treatment group and control group,the control group were used conventional laparoscopic cholecystectomy, and the treatment group were used the umbilical single-port laparoscopic cholecystectomy. Results All cases were cured, The postoperative hospital stay and total hospital costs of the treatment group were significantly less than the control group （P 〈0.05）. The postoperative complication rate of the infection,bile duct injury, back pain, bleeding wound of the treatment group was significantly less than the control group （ P 〈 0.05）. The IgG levels between the two groups compared had no significant difference before treatment, after treatment were significantly decreased （ P 〈 0.05 ） while had significant differences between the groups compared （P 〈 0.05）. Conclusion The umbilical single-port laparoscopic cholecystectomy applied to the treatment of cholecystitis is more conducive to reduce the incidence of postoperative complications and conducive to the re- covery of patients, while has litter effects for the humoral immune function.
