摘    要:目的了解介入治疗过程中放射工作人员所受辐射剂量水平,探讨影响辐射剂量的因素及其减低剂量的途径和方法。方法选择河北省4家省级三甲医院的放射工作人员56人,应用热释光测量法,对不同放射工作人员,不同部位受照体表剂量进行统计分析。结果平均体表受照剂量中,手背部最高,为317.86μGy,其次为甲状腺部位,为233.15μGy。个体受照剂量最高部位为骶尾部脊索瘤栓塞术术者的手部,为3 122μGy。不同手术类型中,肿瘤栓塞术致放射工作人员受照剂量明显高于其他手术。上球管较下球管使放射工作人员受照剂量高。结论介入操作中,放射工作人员受照剂量差别很大,受手术类型、手术复杂程度、术者的操作技术等多种因素影响,因此,应采取有效防护措施使其受照剂量合理降低,特别是主动防护。

Abstract:[Objective] To investigate the radiation dose of operators during therapeutic interventional procedures,and analyze the factors which influence radiation dosage as well as the way to decrease the dosage.[Methods]A total of 56 operators from 4 Third Grade Class A hospitals of provincial level were selected.Theromoluminescence was used to measure the X-ray radiation dose of operators in different position and exposed surface dose of different body parts.[Results] The highest average dose of skin of operators was the dorsum of the hand(317.86 μGy),followed by thyroid gland(233.15 μGy).The highest individual dose of skin was 3122 μGy at dorsum of the hand in sacrococcygeal region of operators.Operator dose in tumor embolization operation was higher than others.The accumulate dose of patients by using the over-tube X-ray machine was higher than the under-tube.[Conclusion]The difference of operators dose are greater during therapeutic interventional procedures which are due to kinds of operation,complexity of operation,the handing technique of operators and so on.It is necessary to optimize radiation dose by adopting dose reduction techniques so as to maintain radiation dose as low as reasonable,especially the active protection.
