摘    要:目的 探讨胸腰段脊柱爆裂骨折的瞬态损伤机制和结构破坏与能量吸收的变化规律,明确L1椎体爆裂骨折的失稳机制及损伤阈值。方法 对18例新鲜尸体的胸腰段脊柱标本进行了不同能量的纵向撞击,实时采集和量化分析损伤节段的参数,结合节段的影像学,解剖病理和三维运动,综合分析节段的损伤状况。

Abstract:Objective To investigate the kinetic relationsnip between struCtural damageand impact energy absorption and the dgnamic mechanism of thoracolumbar burst fracturewith quantitative analysis on various injured segments,and to explore the instability mecha- nism of L1 vertebral burst fracture and its injury threshold. Methods Eighteen humanthoracolumbar spines taken from cadavers between the age of 22 and 34 years were impactedlongitudinally with different energy levels. Through simulation of the real process of thora- columbar bud fracture and use of modem dynamic techniques,the study was made bythree-dimensional analysis of the injured segments with stereophotogrsmndry in combinationof transient PhySical parameters,anatomy and image. Results The impact force and velocitytO the thoracolumbar segments of the human spine significantly increase but action durationof the force was shortened along with the increase of impact energy. The injury seventy of L1vertebra was proportional to the hobo force. The toot with moderate amount of energycould lead to obvious instability of the segments in the ,ovement of fateral bent and rotation.And the impact with high energy could lead to significant instability of the spine in alldirection movementS. Conclusions The energy and impact force might be used as paramerers forjudgement of injury threshold, and the impact velocity, acceleration, vibration velocity anddisptacemat might be used for evaluation of injury limitation.
