DMF方案与FAM方案治疗晚期胃癌的临床观察

 目的　探讨晚期胃癌应用DMF方案及EAM方案的疗效与毒副作用。方法　1995年1月至1998年12月用DMF方案(DDP、MMC、5-Fu)与FAM方案(5-Fu、ADM、MMC)治疗晚期胃癌56例。结果　DMF方案治疗28例，有效率(CR+PR)53.6%。FAM方案治疗28例，有效率(CR+PR)57.1%。两组疗效经x2统计学处理无显著差异性。两组毒副反应均有消化道反应和骨髓抑制，DMF组的消化道反应较FAM组重，而骨髓抑制较FAM组轻。FAM组有明显的脱发，2例出现心脏毒性反应。DMF组无脱发及心脏毒性反应，DDP的肾毒性经水化后得以预防。结论　DMF组疗效与FAM组相似，毒副作用经对症处理均可耐受，无脱发及心脏毒性，价格较低。

Study on the effects of DMF and FAM regimens in the treatment of advanced gastric cancer

Objective To study the effect and toxicity of DMF regimen and FAM regimen in the treatment of advanced gastric cancer.Methods From January 1995 to December 1998,56 patients were treated with DMF regimen and FAM regimen.Results In 28 patients treated with DMF regimen,the total response rate was (CR PR)53 6%. In 28 patients treated with FAM regimen,the total response rate was (CR PR)57 2%.The response rates in both groups were handled statistically with x 2( P >0 05).There were no apparent differences.The toxicity in DMF and FAM group all have maucea,vomithing and myelosupperession.Gastrointestinal toxicity in DMF group was more severe than that in FAM group and the myelosupperession was mild.There were apparent baldness in FAM group,and 2 patients hnd cardiac events.The nephrotoxicity in DMF group can be prevented by hydration.Conclusion The effects in DMF group were similar to the FAM group,and the toxicity was tolerable.There were no baldness,cardiac events and the price was low.So we suggest that the DMF regimen is worthy to be recommended to patients with advanced gastric cancer.
