腰椎脊神经后内侧支阻滞治疗腰椎后关节紊乱综合征的疗效观察

目的评估C型臂X线透视机引导下腰椎脊神经内侧支阻滞治疗腰椎后关节(LFJ)紊乱综合征的疗效。方法将120例LFJ紊乱综合征患者随机分为口服双氯芬酸钠药物组(M组)、C型臂X线透视机定位下关节内注射组(J组)和C型臂X线透视机定位下腰椎脊神经后内侧支阻滞组(N组)各60例,采用疼痛模拟评分法(VAS)和活动功能评分法评价3组疗效。结果 M组有效率73%,J组有效率92%,N组有效率93%,J组和N组疗效优于M组。结论 C型臂X线透视机引导下脊神经后内侧支阻滞及关节内注射治疗能有效缓解腰椎小关节源性疼痛,改善腰椎活动度,是有效治疗腰椎小关节骨性关节炎的方法。腰椎脊神经后内侧支阻滞对LFJ紊乱综合征能起到很好的治疗作用。

Curative effect observation of lumbar spinal nerves medial branch blocking on lumbar facet joints dysfunction syndrome

Objective It is to evaluate the curative effect of lumbar spinal nerves medial branch blocking on lumbar facet joints(LFJ) dysfunction syndrome under C-arm X-ray machine conducting.Methods180 patients with LFJ dysfunction syndrome were divided into diclofenac sodium taken orally group(group M),intra-articular injections under C-arm X-ray machine conducting(group J) and lumbar spinal nerves medial branch blocking under C-arm X-ray machine conducting(group N),in which there was 60 cases.The curative effects of the three groups were evaluated with visual analogue scale(VAS) for pain and activities of the functional scale.Results The effective rate was 73% in group M,92% in group J and 93% in group N.The curative effects of group J and group N were both better than that of group M.Conclusion Lumbar spinal nerves medial branch blocking and intra-articular injections under C-arm X-ray machine conducting can effectively release the pain of LFJ and improve lumber activity.It is the effective treatment for LFJ osteoarthritis.Lumbar spinal nerves medial branch blocking can play a very good therapeutic effect on LFJ dysfunction syndrome.
