经伤椎椎弓根钉固定治疗胸腰椎爆裂性骨折

目的探讨经伤椎椎弓根钉固定治疗胸腰椎爆裂性骨折的可行性及临床疗效。方法经伤椎椎弓根钉固定治疗23例胸腰椎单节段爆裂性骨折患者。结果患者均获随访,时间6～18（12±3.1）个月。骨折椎体均愈合,愈合时间3～6个月,伤椎置入椎弓根钉无神经损伤,术后伤椎高度较术前得到明显恢复,后凸畸形角度明显改善,生理弧度恢复。末次随访时伤椎高度丢失不明显。结论采用经伤椎椎弓根螺钉短节段固定是一种维持胸腰椎爆裂性骨折复位的有效方法。

Preliminary report on short-segment fixation of thoracolumbar burst fractures using pedicle fixation at the level of the fracture

Objective To investigate the feasibility and the clinical outcome of short-segment fixation of thoracolumbar burst fractures using pedicle fixation at the level of the fracture.Methods 23 single thoracolumbar burst fractures were reduced and fixed by using pedicle fixation at the level of the fracture.Results All the patients were followed up.The mean follow-up period was 12±3.1 months with a range from 6 to 18 months.Bony union was observed in the all fractures after 3～6 months.No nerve injury was observed.The Cobb angle and anterior column restoration were achieved.At the end of the follow up,the anterior height of the injured vertebrae were not lost significantly.Conclusions It is an effective method to deal with the thoracolumbar burst fractures with short-segment fixation using pedicle fixation at the level of the fracture,which can help to correct the kyphosis and maintain the reduction.
