乳腺癌腋窝淋巴结清扫术中保留肋间臂神经92例临床分析

目的:探讨乳腺癌术中保留肋间臂神经的方法及临床意义。方法:92例乳腺癌患者行改良根治术69例,保乳术23例,手术中保留肋间臂神经74例,切除肋间臂神经18例,术后对患者上臂内侧感觉功能进行随访观察。结果:保留肋间臂神经74例中术后患侧上臂内侧及腋部皮肤感觉正常66例(87.5%),感觉异常8例(12.5%);而切除肋间臂神经的18例患者均有感觉异常(100%)。结论:在乳腺癌改良根治术中保留肋间臂神经可明显减少术后患侧上臂内侧感觉障碍的发生率,有助于提高患者的生活质量。

Clinical analyzing of preserving intercostobrachial nerve during axillary lymph nodes dissection for breast carcinoma

Objective: To study the methods and clinical significance of preserving intercostobrachial nerve(ICBN) during the axillary lymph nodes dissection(ALND) for breast carcinoma.Methods: Sixty-nine cases of 92 patients with breast carcinoma underwent modified radical mastectomy,23 cases underwent breast conserving surgery,74 cases preserved ICBN in the operation,18 cases excised ICBN.The sensory function of medial sides of the upper arms was observed in follow-up.Results: Sixty-six cases(87.5%) out of 74 cases with ICBN preserved had normal feeling in medial side arm and axillary skin,8 cases(12.5%)had abnormal feeling.18 cases excised ICBN all had abnormal feeling(100%).Conclusion: Preserving ICBN may reduce the incidence of abnormality of sensory function of the medial sides in the modified radical mastectomy for breast carcinoma and improve the quality of life of patients.
