手术治疗胃溃疡的临床观察

目的探讨采用3种不同术式治疗胃溃疡的疗效及并发症情况。方法选择大英河边中心卫生院2006年1月至2010年1月均经内镜、影像学及病史诊断为胃溃疡患者60例作为研究对象,依据手术方式的不同分为A组:BillrothⅠ式胃空肠吻合术;B组:BillrothⅡ式胃空肠吻合术;C组:Roux-en-Y胃空肠吻合术各20例。观察比较3组的临床疗效及并发症的发生率。结果 3组的临床有效率比较,差异无显著性。3组术后并发症主要为输入袢梗阻、吻合口溃疡、倾倒综合征、伤口感染等,其中B组术后并发症发生为70.0%,明显高于其他两组并发症的发生率,差异有显著性(P<0.05)。结论 BillrothⅠ式胃空肠吻合术疗效与其他术式相似,但并发症发生率较低,值得临床推广应用。

Clinical Observation of Surgical Treatment of Gastric Ulcer

Objective To study the surgical treatment in three different gastric ulcer and complications.Methods In Hebian central hospital of Daying in January 2006 ~ January 2010 were confi rmed by endoscopy,imaging,and medical history of 60 cases diagnosed as gastric ulcer patients as research subjects according to surgical procedure were divided into A group： Billroth Ⅰ-type gastrojejunostomy;B Group： Billroth Ⅱ type gastrojejunostomy;C Group： Roux-en-Y gastrojejunostomy in 20 cases each.Three groups were observed and compared the effi cacy and incidence of complications.Results Three groups clinical effi ciency,the difference was not signifi cant.Three groups of postoperative complications for the input loop obstruction,anastomotic ulcer,dumping syndrome,wound infection,postoperative complications including B group and 70.0%,signifi cantly higher than the other two groups the incidence of complications,the difference signifi cant（P0.05）.Conclusion Billroth Ⅰ-type gastrojejunostomy anastomosis and other procedures were similar,but the low rate of complications is worthy of clinical application.
