改良内括约肌侧方位切断术在慢性肛裂中的应用

目的探讨改良内括约肌侧方位切断术在慢性肛裂中的应用。方法回顾性分析2018年4月-2019年8月于阳谷县人民医院接受治疗的75例慢性肛裂患者的临床资料,按照手术方法不同分为观察组40例和对照组35例,观察组给予改良内括约肌侧方位切断术治疗,对照组使用后正方位切断术治疗。比较两组临床疗效、手术情况、恢复情况、并发症发生率。结果观察组临床治疗总有效率为95.00%,高于对照组的80.00%;观察组术后视觉疼痛模拟评分(VAS)低于对照组,术中出血量少于对照组,手术时间短于对照组;观察组术中创面愈合、肛门功能恢复、住院时间均短于对照组;观察组并发症发生率为10.00%,低于对照组的28.57%,差异均有统计学意义(P<0.05)。结论改良内括约肌侧方位切断术治疗慢性肛裂的疗效明显,可以有效降低患者术后疼痛,减少出血量,缩短手术时间,改善恢复情况,减少并发症,值得临床推广应用。

Application of improved internal sphincter amputation in chronic anal fissure

Objective To explore the application of improved internal sphincter amputation in chronic anal fissure.Methods The clinical data of 75 patients with chronic anal fissure who were treated in Yanggu County People’s Hospital from April 2018 to August 2019 were retrospectively analyzed.They were divided into observation group with 40 cases and control group with 35 cases according to different surgical methods.The observation group was treated with improved internal sphincter amputation,and the control group was treated with posterior positive sphincterotomy.The clinical efficacy,surgical status,recovery status and complication rate were compared between the two groups.Results The total effective rate of the observation group was 95.00%,higher than 80.00% of the control group;the VAS of the observation group was lower than that of the control group,the intraoperative blood loss was less than that of the control group,and the operation time was shorter than that of the control group;the wound healing time,anal function recovery time and hospital stay time in the observation group were shorter than those in the control group;the incidence of complications in the observation group was 10.00%,lower than 28.57% in the control group,and the differences were statistically significant(P<0.05).Conclusion The effect of improved internal sphincter amputation in the treatment of chronic anal fissure was obvious,which could effectively reduce the postoperative pain,reduce the amount of bleeding,shorten the operation time,improve the recovery and reduce complications,which was worthy of clinical application and promotion.
