基于“肾主气化”理论探讨肾“损不可复”病机对慢性肾脏病的影响＊

慢性肾脏病的发病率在我国呈逐年上升趋势,由于该病的不可逆性,大部分患者将进展至终末期肾病因各种并发症而死亡。目前现代医学对于慢性肾脏病的防治没有特效方法,多采用对症治疗,终末期肾病多采用透析、肾移植等方法。随着对祖国医学理论的不断挖掘,中医方法可为慢性肾脏病的防治提供新的可能,然而目前中医治疗慢性肾脏病当以扶正为主还是祛邪为主众说纷纭。本文基于对中医“肾主气化”理论的重新认识,旨在探讨“肾失气化”对慢性肾脏病“损不可复”病机的影响,提出恢复“肾主气化”功能为慢性肾脏病中医治疗的核心,强调以“扶阳”之法贯穿本病始终,在邪实证候突出时兼以祛邪,以期对“肾主气化”在慢性肾脏病的治疗上进行理论指导。

Study on the Effects of Pathogenesis of "Irrecoverable Kidney Damage" on Chronic Kidney Disease Based on the Theory of "Kidney Governing Qi Transformation"

The incidence rate of chronic kidney disease is increasing year by year in our country. Due to the irreversibility of the disease, most patients will progress to end-stage renal disease and die from various complications. At present, modern medicine has no specific methods for the prevention and treatment of chronic kidney disease. Symptomatic treatment is mostly used, and dialysis, renal transplantation and other methods are mostly used in end-stage renal disease. With the continuous excavation of traditional Chinese medicine theory, traditional Chinese medicine methods can provide new possibilities for the prevention and treatment of chronic kidney disease. However, at present, there are different opinions on whether traditional Chinese medicine should focus on strengthening health or eliminating the pathogenic factor in the treatment of chronic kidney disease. Based on the new understanding of the theory of "kidney governing qi transformation", the purpose of this paper is to explore the effect of "kidney losing qi transformation" on the pathogenesis of "irrecoverable kidney damage" on chronic kidney disease. It is suggested that the recovery of the function of "kidney governing qi transformation" is the core of the treatment of chronic kidney disease. It is emphasized that the method of "supporting yang" throughout the disease of whole course, and dispels the pathogenic factor when the syndrome is notable, in order to provide theoretical guidance for the prevention and treatment of chronic kidney disease.
