关节腔负压冲洗配合注射玻璃酸钠对膝骨性关节炎的远期疗效观察

目的探讨关节腔负压冲洗配合注射玻璃酸钠对膝骨性关节炎的远期疗效。方法膝骨性关节炎患者80例，分为负压冲洗加注射玻璃酸钠组（A组）和单纯注射玻璃酸钠组（B组）。冲洗液含复方氯化钠，50％硫酸镁，庆大霉素，糜蛋白酶，A组关节腔置冲洗管和吸出管，冲洗液进入关节腔同时开启负压吸引。3d后，将玻璃酸钠注入膝关节腔。采用Lysholm膝关节功能评分评估患膝的功能恢复情况。结果两组治疗第1个月后，功能均较术前有明显改善（P〈0．05），6个月和1年后A组疗效改善，与术前相比仍较明显（P〈0．01），6个月和1年后A组较B组有明显改善（P-〈0．05）。结论膝关节腔负压冲洗配合玻璃酸钠注射较单纯玻璃酸钠注射对膝骨性关节炎有更好的远期疗效。

Long-term efficacy of intra-articular negative pressure irrigation combined with injection of sodium hyaluronate in the treatment of knee osteoarthritis

Objective To observe the long-term efficacy of intra-articular negative pressure irrigation combined with injection of sodium hyaluronate in the treatment of knee osteoarthritis. Method Eighty patients with knee osteoarthritis were divided into two groups. Patients in group A were treated with intra-articular negative pressure irrigation combined with injection of sodium hya- luronate; patients in group B only with injection of sodium hyaluronate. Irrigation fluid included Ring- ers solution, 50% magnesium sulfate,gentamycin and chymotrypsin. Double tubes （in and out） were inserted into the articular cavity, and negative pressure suction was turned on to irrigate the cavity at the same time. Sodium hyaluronate was injected into the articular cavity 3 days later（Group A）. Results The articular function of all the patients in the two groups was improved after 1 months （P〈0.05） and the efficacy in group A was more improved（P〈0.01） after six and one year. The efficacy in group A was better than that in group B（P〈0.05）. Conclusion With the mothod of intra-articular negative pressure irrigation combined with injection of sodium hyaluronate, a better long-term efficacy is received than that with sodium hyaluronate injection only in the treatment of knee osteoarthritis.
