胃癌CD44v6表达及其临床意义的研究

摘    要:
探讨胃癌CD44v6的表达,评价其表达对胃癌进展程度和转移的诊断意义,以及对手术预后的影响。方法收集胃癌手术标本85例,相应的癌转移淋巴结标本47例,应用S-P免疫组化染色技术检测CD44v6蛋白在上述组织中的表达。结论:对胃癌组织进行CD44v6蛋白的检测,有助于预测胃癌进展程度和淋巴结转移的诊断,以及评估胃癌患者的预后。

Abstract:
Objective: To evaluate the expression of CD44 splice variant v6 in gastric cancer and its role in the estimation of tumor progression, metastasis as well as prognosis. Methods: The expression of CD44v6 protein was determined immunohistochemically in 85 surgically resected primary tumor specimens and 47 excised metastatic lymph nodes of gastric cancer using murine exon v6-specific monoclonal antibody. Results: The positive rate of CD44v6 immunoreactivity was 74.1%(63/85) and 83.0%(39/47)in primary gastric cancer and metastatic lymph nodes respectively. The positive rate of CD44v6 immunoreactivity was 80.0%(44/55) and 47.6%(10/21) in the intestinal-type and diffuse-type tumors respectively as classified by Lauren.The positive rate of CD44v6 immunoreactivity in the intestinal-type cancer was higher than that of the diffuse-type significantly (P< 0.05). The presence of CD44v6 protein was correlated with the lymph node involvement ratio (ie the ratio between the number of excised and involved lymphnodes). The staining intensity of CD44v6 was relatedwith the UICC TNM classification. Patients with CD44v6 positive tumor had low 3-year survival rate (P< 0.05). Conclusion: Determination of CD44v6 protein by immunohistochemistry might be useful in assessing tumor progression and lymphatic metastasis and in predicting prognosis of gastric cancer. In addition, the results of our study support the hypothesis that diffuse-type and intestinal-type carcinoma of the stomach arise from different precursor cells of the normal stomach mucosa and progress by different pathways.
